While the protection system design of the these legacy controls is still
applicable today, system optimization now includes additional requirements
such as full power metering; SCADA and DA applications; advanced data
logging through oscillography and event recorder; and extensive user
customization. Feature enhancements continue to be software driven and
downloadable to existing hardware.
Cooper Power Systems offers this recloser control technology at an affordable
price with the PerForm 6 retrofit program. The PerForm 6 retrofit program
enables Cooper Power Systems customers to replace specified legacy recloser
controls with new Form 6 pole mount, yard mount, or triple-single pole mount
controls for a special price with an old Form 3A, Form 4A, Form 4C, FXA, FXB,
or Form 5 control in exchange.

returned in non-working condition.”
3.When your Form 6 control, PerForm 6 program letter, and packing list arrive,
prepare your retired control for shipment to Cooper Power Systems.
4.Follow the instructions in the letter to fill out the packing list.
5.Box up your retired control and attach the packing list to the outside. Follow the
shipping instructions outlined in the PerForm 6 program letter to send your retired
control back to Cooper Power Systems.

Limitations
• All Form 3A, 4A, 4C,

the PerForm 6 retrofit
program.
• Additional accessories
beyond control base
catalog number
KME6P2A, KME6Y2,
or KNTS6 are added
at list. The Form 6 pole
mount control for use with
control-powered NOVA
reclosers is not included
in the PerForm 6 retrofit
program.
• Cooper Power Systems
reserves the right to
assess a surcharge of
$2000.00 for each control
not returned in 90 days or
returned in non-working
condition.
• Cooper Power Systems
will assume the cost of
returning retired control
units to the factory only
if shipment is returned
via the carrier noted in
the PerForm 6 program
packing list you receive.
• PerForm 6 program
discounts can not be
applied to previous orders
or be combined with other
discounts.
• Cooper Power Systems
reserves the right to
discontinue this program
without further notice.
