This Procedure must be implemented immediately any time a VoIP phone is moved from one room to another room. 2.0 Governing Policy Number/Document Name 6.6 911 Location Database Updates Effective Date 03/18/2013 3.0 Procedure Requester: Individual moving VoIP Phone: 1. 2. Complete form 6.6.F.1 titled “911 Location Info – Update Form” available from the IT Forms Library at: http://www.emich.edu/it/help/forms_library/index.php . Submit the form via e-mail to phonehelp@emich.edu. Telecommunications Specialist or Network Systems Engineer: 1. 2. 3. 4. Receive properly completed form. Within 24 hours of receipt, access Location Database System and complete update(s) as requested in the submitted 911 Location Database Update Form(s). E-Mail requestor (as listed on submitted form) confirming updates are complete. File form in appropriate departmental file. 4.0 Responsibility for Implementation The Director of Network and Systems Services is responsible for the implementation of this procedure. 5.0 Definitions Term VoIP Phone Definition A phone that provides telephone service using the set of protocols commonly referred to as Voice over Internet Protocol (VoIP).
