CONFERENCE REGISTRATION
This activity is designed for primary care physicians, cardiologists,
anesthesiologists, emergency medicine physicians, nurse practitioners,
physician assistants, registered nurses, electrophysiology technicians and
biomedical engineers.
Please register early – space is limited.
EDUCATIONAL NEED
Tuition includes continental breakfasts, refreshment breaks, lunch,
electronic syllabus, and an online certificate of attendance. Tuition may
be paid by check, American Express, Discover, MasterCard or VISA.
Cash is not accepted. Conference materials cannot be guaranteed unless
registration is received by May 14, 2016. The Office of Continuing
Medical Education reserves the right to cancel this conference. In such a
case, a full refund will be given.
As a result of previous evaluations and our community physician annual
needs assessment survey, the need for cutting edge information about
electrophysiology has been identified. Methods to diagnose treat and
manage patients with arrhythmias and cardiac devices continue to change
rapidly. This conference is designed to change physician and other health
care provider’s competence and/or performance in practice and/or patient
outcomes. This year’s one day conference will provide practitioners with
state-of-the-art tools to help them provide better diagnosis, treatment and
management options for their patients. Faculty presenting and interacting
with participants at the activity have been selected on the basis of their
comprehensive clinical experience and academic excellence.
EDUCATIONAL METHODS
• Lectures with Q and A will be used to facilitate the dissemination of
up to date information.
• Case studies and discussion will highlight conditions commonly seen
in clinical practice.
• Interactive discussions are included to facilitate a more in-depth
understanding of clinical issues.
• Hands-on skill building workshops are included to provide attendees
with the opportunity to simulate in-lab procedures.
EDUCATIONAL OBJECTIVES
At the end of the activity, participants should be able to:
• Recognize evaluation and management of arrhythmias.
• Interpret indications and follow-up of cardiac implantable
electronic devices.
• Describe modern therapy options for arrhythmia and heart
failure management.
Tuition is $199 for physicians and $179 for all others. Confirmation
of registration will be sent within two weeks after receipt of
registration form. The fees will increase after April 1, 2016.
Registration forms received without payment will not be processed
until payment is received, and payment must follow within 10 days.
Early discount rates are honored if payment is received by the date noted
on the registration form. After that date, the late fee is applied.
On-site Fees: At door registrations will be charged at a higher rate.

Cancellation and Refund Policy:
A refund of tuition, less $75 administrative fee, will be allowed if requested
in writing by May 14, 2016. No refunds will be provided after this date.
ELECTRONIC EVALUATION AND CREDIT
The link to your electronic syllabus, evaluation and certificate of credit
will be available for 30 days after the last day of the conference. Once
you have completed the online evaluation your certificate will be
available to print. However, after 30 days this link will expire and
these items are no longer available online. You must contact the Office of
CME to receive your certificate. A $15 administrative fee will be applied.

SYLLABUS
The electronic syllabus will be available to pre-registered attendees
online prior to the conference for downloading and printing. A flash
drive containing the syllabus will be distributed at the conference. There
will be no printed syllabus for this activity.
A block of rooms is being held for conference participants at a rate of $109
single or double occupancy plus hotel and sales tax (extension dates may be
available). After 5/14/16, reservations will be on a space and rate available
basis. Reservations should be made directly with the hotel. Please be aware
that a limited number of rooms have been reserved and the hotel will likely
sell out prior to the reservations deadline of 5/14/16.
By staying at the host hotel, you help the University of California, Davis
meet its contractual obligations with the Hilton Hotel Arden West and keep
future registration fees reasonable. Please consider that when making your
accommodation decisions. We encourage you to stay at the host hotel.
