JOB SUMMARY - Case Management Liaison Works collaboratively with Case Management staff and other health care team members to assist in the execution of a plan of care to facilitate a safe discharge in a timely manner. Carries out assigned tasks to complete a safe discharge plan, including distributing SNF inquiries; locating and arranging a skilled or subacute placement; ordering durable medical equipment. Develops and maintains a knowledge of the referral network to enable a smooth transition of patients to needed post-acute services. Minimum Required: Associates Degree in Social Work or completion of an LVN program. Two years’ experience in case management/assessment and referral. Current CA LVN license for incumbents without Social Work degree. Current BLS certification. PREFERRED: Bachelor’s Degree in Social Work or healthcare related field.
