Social Worker – Acute
The Social Worker assists patients who present with complex psychosocial needs, and works
collaboratively with the Case Management team, health care providers, and physicians. Intervenes
to help the patient and/or family understand, and cope with personal, emotional and
biopsychosocial issues affecting hospitalization and the delivery of care. Interventions include
comprehensive assessment and referral, crisis intervention, and care management. Collaborates
within a multi-disciplinary team, and community agencies to facilitate a safe discharge plan through
the continuum of care. This position promotes individual professional growth and development by
providing direct supervision of interns.
EXPERIENCE AND EDUCATION
Minimum Required: Master’s degree in Social Work (MSW). One year Case Management experience in a clinical setting.
Preferred: Two years of acute Case Management experience in a healthcare/social service agency within the past five years.
LICENSES/CERTIFICATIONS
Minimum Required: Current BLS certification. California Children’s Services (CCS) paneling within 6 months of hire or entry into
job.
Preferred: Licensed Clinical Social Worker (LCSW), Critical Incident Stress Management (CISM) certification, Accredited Case
Management (ACM) certification.
