Incident Report After recovery, the Disaster Supervisors will gather reports and feedback from Team Leaders and others involved to draft a report, including photographs or other visual documentation. The report should give the date and location of the emergency, describe the damage in detail, and give an account of recovery and salvage efforts. EPWG will examine the report and consider the effectiveness of the Disaster Plan (what worked? what didn’t? what needs to be revised?). The final report that EPWG will submit to the Director of Libraries will consist of the original Disaster Team report, EPWG’s critique of the effectiveness of the Disaster Plan, and an evaluation of the sources of supplies and all off-site facilities used. 03/19/03
