Transit(Pass(Number:(___________( ( Saskatoon Transit Eco Pass Program UNIVERSITY OF SASKATCHEWAN (the “Employer”) And _____________________________ (the “Employee”) 1. Employee Obligations 1.1. The Employee agrees to purchase one Saskatoon Transit Adult Pass each month at the discount of 20% off the regular Pass fare plus the applicable Employer’s discount of 20%. The Employee acknowledges that the Employer’s discount is a taxable benefit. 1.2. The Employee agrees that they cannot and will not give away, resell or barter the Passes for any consideration and acknowledge that the Employee must be the primary user of the Pass. 1.3. The Employee agrees to permit payroll deductions to pay for the Pass for a minimum of twelve (12) consecutive months and acknowledges and agrees that one cannot exit the Program prior to the completion of the twelve (12) consecutive month period. The Employee agrees that they will continue in the Program until proper notice is provided by the Employee to the Employer. 1.4. The Employee acknowledges that if the Employer cannot obtain and maintain the minimum number of three (3) Employees as required by Saskatoon Transit, the Program may be suspended by Saskatoon Transit, until the minimum number can be attained and maintained by the Employer. 1.5. The Employee acknowledges that this program offered by the Employer may be terminated by the Employer at any time. 2. Exit and Leave from the Program 2.1. The Employee can exit the Program no earlier than their twelfth month anniversary date by providing written notice to staff.facultyparking@usask.ca at least one month in advance of the date of exit from the Program. 2.2. If the Employee exits the Program and wishes to participate in the Program again, the Employee must re-apply and commit to the minimum. 2.3. The employee may obtain leave from the Program for reasons resulting from a change in location of employment or employee’s residence, maternity/paternity leave, short-term or long-term illness, WCB disability, or Employer-authorized education or extended leave. 2.4.1 Any such leave must be approved by the employer and such leave will exempt the Employee from the minimum required twelve (12) consecutive month participation in the Program. 2.4.2 The Pass must be returned to the Employer Program Coordinator within five (5) days of the start of the leave. If the Pass is not returned within five (5) days of the start of the leave, the Pass will be terminated with Saskatoon Transit and the Employee will have to re-apply when they return. 2.4.3 Employee holidays are not a valid reason for temporary leave from the Program. 3. Lost, Stolen or Confiscated Passes 3.1 Saskatoon Transit shall not be liable to reimburse any Employee directly for lost, stolen or confiscated passes purchased by the Employer. The Employee must contact the Employer Program Coordinator for the procedure to have lost or stolen cards replaced. Revised(August(2014( Transit(Pass(Number:(___________( ( 3.2 If a Pass has been damaged, the Employee may obtain a replacement for the damaged Pass if the remnants of the damaged Pass are identifiable as a valid Pass or if the Employee has registered the Pass with Saskatoon Transit’s Customer Service. The Employee must contact the Employer’s Program Coordinator to exchange a damaged Pass. 4. Saskatoon Transit Passenger Conduct 4.1 The Employee acknowledges and agrees that the Pass remains the property of Saskatoon Transit and its use can be revoked and/or confiscated for misuse, tampering, damaging of the Pass or any fraudulent use. No cash refunds will be permitted. Lost or stolen Passes will only be replaced if they have been registered with Saskatoon Transit by the Employer. 5. Agreement to Program Terms and Conditions 5.1 By submitting an application to participate in the Program, the Employee acknowledges that he/she has read and agrees to comply with these Program Terms and Conditions. 5.2 By submitting an application to participate in the Program, the Employee acknowledges that he/she agrees to participate in Program surveys conducted by Saskatoon Transit and the Employer. The Employee agrees to allow all personal information contained in his/her application for the Program to be collected and made available to the City under authority of The Freedom of Information and Protection of Privacy Act or The Local Authority Freedom of Information and Protection of Privacy Act, as the case may be which will be used solely to administer and evaluate the Program. This information is protected by the privacy provision of The Freedom of Information and Protection of Privacy Act or The Local Authority Freedom of Information and Protection of Privacy Act, as the case may be.
