Request for BADM 595/CSCI 471 Research Proposal
1. This form must be completed in full. The Associate Dean for Administration will not create
the course until in receipt of the form, completed through the dean’s signature.
2. The faculty member will forward a copy of all graded material to the Associate Dean for
Administration.
3. The Associate Dean for Administration will be responsible for maintaining an archival file
for this course, including at least, this form, the syllabus, the student graded material, and the
final grade assigned.
