Alpha Psi LambdaNational, Inc. Scholarship Application History Since 1985 Alpha Psi Lambda has been the first and largest co-ed fraternity in the nation. Zeta chapter was founded at the University of Illinois at Chicago in 1993, and since then has worked for the needs and concerns of Latino/a students. As a Latino based organization, we understand how difficult it is to pay for higher education, especially when one might not receive federal funds. This is why, as an organization, Alpha Psi Lambda will recognize one new student in good academic standing who demonstrates outstanding leadership skills regardless of residential status. In addition, preference will be given to individuals who demonstrate financial need. The Noble brothers and sisters of Alpha Psi Lambda at Zeta Chapter are proud to announce our first Alpha Psi Lambda scholarship for $1000. • To be eligible, a student must fulfill the following criteria: Must be Latino/a Must be a first generation student Not receive funds from FAFSA Have a GPA of 3.0 or above on a 4.0 scale Must be a high school senior or be a current student at an accredited U.S. community college or two-year institution Plan to attend University of Illinois at Chicago in the fall of 2015 as a full-time student • • • • Requirements A copy of your high school/college transcript Must provide proof of enrollment into the University of Illinois at Chicago One letter of recommendation A copy of award letter/summary • • • • • Information Name: Street Address: City, State, ZIP Code: 1 Home Phone: Work Phone: E-Mail Address: Race/Ethnic Group: Number of household members: Estimated family income: Estimated Tuition: Estimated family contribution: High School: GPA on a 4.0 scale: Extracurricular Activities/Community Service Date Organization Leadership position Contact Information 2 If applicable, any scholarships you have been awarded Scholarship name Amount Personal Statement Create a personal statement 250 words or less answering the following questions: 1.
