Grade Release Permission Form
I hereby give permission to the UW-La Crosse Registar’s Office and FSL Advisers to release my
semester and cumulative GPA to the chapter, advisers and/or the FSL Advisers working with my
organization. The FSL adviser may, in turn, release my semester and cumulative GPA to my chapter’s
national organization, academic chairperson, president, or chapter advisers requesting them for
academic compliance, survey, or awards purposes.
