Not sure what insurance plan you have?

LabCorp is our preferred provider for lab
work paid at 100%.

All services/lab work performed at
Rose-Hulman Health Services are paid at
100%.
Flexible Spending Accounts
Buy Up (Office co-pays are $20/$30)

Unreimbursed Health Care Spending
Accounts can be used for eligible expenses for
you or any dependents on your insurance,
including your spouse.

Please save all receipts for eligible expenses in
case UMR requests them.
Consumer Choice (Office co-pays are $25/$35)
Health Insurance Deductibles
Deductibles for both the Consumer Choice and Buy Up plans
reset on January 1. This includes the $500 upfront allowance 
available with the Consumer Choice plan.
SimplyWell

FSA participants have a grace period to incur
expenses from July 1—September 15 for the
previous plan year. Grace period doesn’t apply
to Dependent Care (child care).

Dependent Care Accounts are for daycare
expenses only.

Participants can elect up to $5,000 for the plan
year.
If you elect the Consumer Choice plan, you may enroll in
SimplyWell effective July 1, 2016.
If you elect the Buy Up plan, you and your spouse (if
applicable) are required to enroll in SimplyWell effective July
1, 2016.
Participants can elect up to $2,550 for the plan
year (July 1—June 30).
YMCA Membership
Dental/Vision

The dental plan year is July 1 - June 30.

The annual limit is $1,500 per person. This
includes routine cleaning visits. There is no
family limit.

Participants have a lifetime
orthodontic benefit of $1,000.

YMCA memberships will be provided at no cost to the
employees or their family as long as the employee utilizes
one of the facilities (either one of the YMCAs or the SRC)
on a regular basis and averages six visits per month over
the ten week academic quarter.
Employees must scan their YMCA key cards at either
YMCA location or at the SRC front desk for the visit to
count toward the utilization requirement.
Open Enrollment Information Sessions
Pamela Pahnke, HR Generalist, will be hosting three Q and A sessions on benefits before open enrollment begins on May 1.

Twenty-first Annual Mini Medical School Series
“The mission of the Mini-Medical School is to provide an interactive forum free of charge to
discuss medical training, medical procedures, current medical news and medical concerns to the

Adult LEAF (Lifestyle Education And Food) Program: This 8 week class can help you survive and thrive.
You will learn to eat healthy, exercise moderately and practice stress management techniques. LEAF can teach you how to
reverse heart disease, diabetes, hypertension, gout, and high cholesterol. Classes for this program will be held on the RoseHulman Institute of Technology’s campus. SimplyWell participants may earn wellness points for participating.
Youth LEAF (Lifestyle Education And Food) Program: This program is for ages
8-12. Fun activities to
teach good nutrition, exercise, and stress management.
The Power of Food for Cancer Prevention and Survival Class: This is a 4 week nutrition class where you will learn
about healthy eating specifically for cancer prevention.
The Power of Food for Diabetes Prevention and Treatment: This is a 4 week nutrition class where you will learn
about healthy eating specifically for diabetes prevention and treatment.

Healthy Recipe
Chocolate-Cherry Snack Bars
From EatingWell: January/February 2011
These cereal bars are chewy, crunchy and delicious with good-for-you seeds, nuts, fruit and little explosions
of chocolate. We like the flavor of dried cherries or cranberries, but any coarsely chopped dried fruit will
work.
16 bars | Active Time: 15 minutes | Total Time: 1 hour 50 minutes (including 1 hour cooling time)
Ingredients










2 1/2 cups unsweetened puffed wheat cereal
1/2 cup pecan halves, chopped medium-fine
1/3 cup salted roasted pepitas (see Notes)
1/4 cup dried cherries or dried cranberries, coarsely chopped
2 tablespoons sesame seeds
1 tablespoon ground flaxseeds (see Notes)
1/2 cup honey
1/2 teaspoon vanilla extract
1/8 teaspoon salt
1/2 cup mini semisweet chocolate chips or finely chopped bittersweet chocolate
Preparation
1. Position a rack in lower third of oven; preheat to 300°F. Line an 8-inch-square pan with parchment
paper, letting it overhang on two opposite sides.
2. Toss cereal, pecans, pepitas, cherries (or cranberries), sesame seeds and ground flaxseeds in a large
bowl.
3. Combine honey, vanilla and salt in a small saucepan. Warm over medium heat, stirring, until the honey
is more fluid and the salt is dissolved. Pour the honey mixture over the dry ingredients and fold until
everything is moistened and sticky. Let cool for 5 minutes. Fold in chips (or chopped chocolate) until
evenly distributed. Scrape the mixture into the prepared pan and spread evenly with a fork. Using the
back of the fork, press the mixture very firmly all over. (Alternatively, cover with parchment paper and
press firmly all over.)
Bake until the top is golden brown, about 35 minutes. (If in doubt, take it out so the honey does not burn.)
Run a knife along the unlined sides of the pan to detach the bars. Let cool in the pan on a wire rack to
room temperature, about 1 hour. Use the ends of the parchment to lift the bars from the pan. Gently peel
off the parchment. Use a heavy sharp knife to cut into 16 bars or squares.

Notes: Hulled pumpkin seeds, also known as pepitas, are medium-dark green and have a delicate nutty flavor.
They are sold salted, roasted and raw, and can be found in the natural-foods section of many supermarkets.
Look for ground flaxseeds (or flaxmeal) in the natural-foods section of large supermarkets or in natural-foods
stores. Store in the refrigerator or freezer.
