2. Log in to S.C.O.T.S. to print a Degree Works Audit for your NEW Program using the What-if worksheet. Make sure the
Include In-Progress Classes box is checked before selecting the Process What-If button.
3. Take this application and a copy of your Degree Works Audit to the departmental representative of your NEW Program as
listed on the attached sheet.
4. The department representative will orient you into the new Program, reevaluate any pre-existing Curriculum Adjustments
made for your current program, and will sign the form in the appropriate space.
5. You must return this approved application to the Office of Records and Registration to have your NEW Program code and
advisor changed in your student record. Please allow 7 to 10 working days for the change to be completed.
