PATIENT RIGHTS & RESPONSIBILITIES All students have the right to humane, competent, and confidential treatment. Students are expected to cooperate and participate in their care by providing accurate information and feedback concerning their condition, treatment, and individual needs. An atmosphere of mutual respect should exist at all times. NOTICE OF PRIVACY PRACTICES As a member of the PASSHE system, Kutztown University follows both Family Educational Rights and Privacy Act (FERPA) and Health Insurance Portability and Accountability act of 1996 (HIPAA) guidelines to protect the confidentially of all student health records. STUDENT HEALTH INSURANCE Eligible students who are not covered by an insurance plan or who wish to supplement their existing plan may purchase low-cost insurance through a university-sponsored plan. Enrollment information and claim forms are available in Health Administration Services and on the web site; www.chpstudent.com EXTENDED ABSENCES The Director for Health & Wellness Services, in collaboration with the Provost’s Office, will initiate notification of the student’s professors in the event that Clinical Services recommend an extended leave of absence for 5 days or more for medical reasons. In the event that classes are missed per the recommendation of an outside provider, the student will need to secure an absence note from that physician. ELIGIBILITY Currently registered students who have paid the health center fee and have submitted a completed health record are eligible for treatment and services. Students who are not 18 years old will require written permission from a parent or guardian for treatment. PHYSICIAN EVALUATION GOALS & PURPOSES Clinical Services is committed to the optimal health of all students through the provision of high quality of care including of: Illness and injury Evaluation of urgent care needs Patient education & primary prevention Cooperative sports medicine Promotion of a healthy campus environment After an initial consultation with a nurse, a physician appointment may be scheduled during the fall and spring semesters. IMMUNIZATIONS (Available upon request) Students who wish to obtain either a measles/mumps/rubella (MMR) or tetanus vaccine may schedule an appointment by calling Health Administrative Services at 610-683-4082. CLINICAL SERVICES The department is an accredited ambulatory care facility; it is not an emergency room. Students are encouraged to schedule an appointment via on-Line Scheduling System or by calling 610-683-4082. “Walk-in” encounters will be triaged for appropriate follow-up (urgent or same-day appointments as necessary). SCHEDULING APPOINTMENTS BY OPEN COMMUNICATOR Students have the ability to schedule their own appointment on-line from the convenience of any computer. Please see the Clinical Services website for instructions. HEALTH EDUCATION Health education is included with every student encounter in Clinical Services. Medication information and teaching sheets related to a particular illness or injury are distributed. In addition, good nutrition and healthy lifestyle living are encouraged. WOMEN’S CLINICAL SERVICES Routine GYN care is provided on site by appointment with a GYN provider at minimal cost. Services include annual exams & pap smears, contraceptive information and treatment of GYN problems. Call 410-683-4082 for appointment scheduling. TB TESTING Group tuberculin testing will be offered in the fall and spring semesters. Contact Health Administrative Services or visit the departmental website for more information about this service. LABORATORY SERVICES Professional nursing staff can perform basic lab work ordered by physicians for a nominal fee. Fees for external laboratory processing are the responsibility of the student or an accepted insurance carrier. The following basic diagnostic testing is available on-site: strep throat screening, screening for urinary tract infections, urine pregnancy testing and STD testing. MEDICATIONS Clinical Services dispenses some medications, as needed, for a small fee. If a non-stocked medication is indicated, a prescription will be written and the student will need to obtain it at a local pharmacy at his/her expense or in accordance with an individual pharmacy plan. SELF-CARE UNIT Clinical Services has a self care counter located in the hallway lobby outside of the department. It is available during hours of operation. Available self care items include: over-the–counter medications, first aid supplies, patient education materials and instruction sheets for typical minor health problems. COUNSELING CENTER REFERRALS Referrals will be made to University Counseling Services for students experiencing acute psychological or emotional distress. EMERGENCY TRANSPORT The University’s Public Safety & Police Services department personnel will respond to emergencies on campus and arrange transportation as needed. If an ambulance service is required, the student will be billed by the ambulance service. ALLERGY INJECTIONS Allergy injections are administered by appointment during designated hours. Students are required to provide prescribed serum and all necessary documentation from their allergist. Visit our website for more information. X-RAY STUDIES Students who may need x-rays will be referred to a local radiology office or emergency room. Fees for these services are the responsibility of the student or an accepted insurance carrier. NON EMERGENCY TRANSPORT Non-Emergency Transport is available to students for urgent care and Doctor’s appointments’, there is a fee for this service. EMERGENCIES During hours of operation, any emergencies that present at Clinical Services will be given first aid until emergency medical services arrive.
