Assessment and Evaluation KPI Target Outcome Actual Outcome Action Plan Immunization Data Processing - volume of service Accurately and efficiently enter student immunization records into CUNYFirst database in a timely manner so incoming Freshman and Transfer students can register at QCC. Attributes of new database system means that the data processing system in Health System takes substantially longer. Staff more cautious in reviewing records because CUNYFirst does not have built in safeguards that SIMS possessed. Continue to work with Continuing New Student Enrollment in obtaining advisement schedules in advance so that students can be contacted and immunizations obtained prior to advisement appointments. Health Services will continue to advise all incoming students and other QCC Departments that immunization records should be brought in at least 48 hours prior to advisement appointments. Health Services continues to use the New York City Citywide Immunization Registry to obtain records in advance. Currently, CUNYFirst does not have the capability to merge New York Department of Education Immunization download, so office staff will continue to manually input data received from a webdrive received from IT. Seasonal staff (winter/spring and summer/fall registration periods)will be requested to assist strained office in data collection and daily office functions. From June 2009 to May 2010, 149 requests have been approved by this office. From June 2010 to April 2011, 103 requests have been approved by Health Services. Decrease in volume caused by students being referred to QCC Counseling as well as students not following through with their medical withdrawal requests. Continue to have Continuing part-time College Assistant and part-time Nurse assist students with medical withdrawals in a timely manner. Medical Withdrawals 1. Assist and advise - volume of service students on the medical withdrawal process and expedite requests. 2. Ensure collaborating departments are versed in CUNY medical withdrawal policy and updates. An additional goal is to promote future knowledgeable health consumers. Compared to last year's report, the office has maintained the same number of events. There has been positive feedback from surveys collected at various presentations and most events have been well attended. 1. Continue to increase Continuing or maintain the number of successful events for next year. 2. Maintain collaborations and networks with other departments and outside agencies to provide insightful presentations. Vaccination Clinics volume of service Provide necessary MMR immunization either through office referrals or on-site immunization clinics in order to achieve 100% compliance and retention objectives. Compared to the previous year of 458 students vaccinated, 322 students were vaccinated from June 2010 to April 2011 for MMR alone. Decrease due to MMR vaccine shortage during the Spring 2011 registration period and records being obtained from the DOE download, Citywide Immunization Registry and contacting student's high schools and medical offices. Attendance and appointment lists are also obtained from Testing and New Student Enrollment so that students can be screened and processed well before their advisement dates. All phases and transfer lists are also sent to the office from Admissions which the office staff also processes. Continue to organize Continuing vaccination clinics in addition to vaccinating students without appointments. Staff will continue to obtain immunization records from the aforementioned sources. Attendance and appointment lists will continue to be requested to try to divert registration delays for incoming students. First Aid Treatment and Counseling/Offcampus referrals volume and quality of service (first aid logs and accident reports) Provide on the spot quality first aid and emergency treatment within the limited resources of Health Services. Full-time Nurse and afternoon part-time nurse provided 2,127 incidences of services for First Aid treatment/Nurse consultation. Continue to provide the Continuing campus community with immediate First Aid treatment and care. The office will continue to have an open door policy where members of the campus community are able to consult with a nurse on personal health issues and concerns. A morning part-time nurse is recommended to cover the office when the full-time Nurse/Director is unavailable. Continue to work closely with the CUNYFirst team to iron out issues that result in delays and inaccurate compliance results. Nursing and Massage Therpay clearances Volume and quality of service Clear 100% of all Nursing and Massage Therapy students for clinic rotations. 1. Continue to review and set up requirements for the proceeding semester at least three months in advance. 2. Maintain new system of submitting requirements to Nursing Department so that professors can distribute requirements to students. Students will have plenty of time to submit completed requirements to Health Services. 3. Work with Massage Therapy to implement a system to have MT students complete clearances in a timely manner. Student Medical Withdrawals Volume of Service Process all medical withdrawals (except those concerning mental health) received in a timely manner. 1. Keep up to date with all changes made to the procedure. 2. Continue to work closely with students who have outstanding appeals. 3. Maintain open communication with Registrar in order to expedite all medical withdrawal requests. Health Insurance Enrollment Volume of service (Daily logs 100% Health Insurance coverage for maintained by Health the QCC student population, Insurance providers) especially QCC student athletes. 51.95% increase in low cost health insurance enrollment as per data provided CUNY Health Stat report Continue to work closely with health insurance providers and Campus Series to ensure viable health insurance opportunities for all students and athletes and decrease any liability for the school.
