STUDENT / PARENT CONTRACT
As a participant in the 21st Century Community Learning Center program, I agree to the following rules and policies:
1. I understand I must attend class sessions and any special workshops offered during the semester. In the event
that I am unable to attend class or workshop, I must notify the site director, peer mentor and course instructor
immediately.
2. I understand the attendance policy is:
a. I may have no more than three unexcused absences. Any more may jeopardize my receiving credit and
a passing grade for the course. Excessive absences any also impact my participation in the program.
b. I may not be late to class.
c. I understand that two latenesses to class count as an absence.
d. I understand that my attendance to class and special workshops will determine whether or not I’m
invited to attend the annual college tour and any other special trips.
3. I understand that I must complete all required course assignments. Failure to do so will have a direct impact on
my grade.
4. I understand that if I am experiencing difficulty with the class work or with any other aspect of the program, I
will notify the site director, peer mentor, and/or my school liaison.
5. I understand that written notification from a parent/guardian must be submitted if I decide to withdraw from
the program.
6. I understand that I must adhere to all QCC campus rules and policies regarding appropriate behavior. Any
behavior that is not aligned with the program’s policies and procedures may result in dismissal from the
program.
7. I understand as a parent/guardian of a participant in the QCC 21st CCLC program, I am expected to attend the
semester’s mandatory orientation session as well as any other parent workshops or activities provided.

understood and accepted.
