Ladies and Gentlemen, READ THIS DOCUMENT IN FULL, FOLLOW THE INSTRUCTIONS PRECISELY AND COMPLETE THE REQUIREMENTS WITHIN THE TIMEFRAME PROVIDED! Each of you needs to complete a Marine Select survey, after which you will report to your Marine Mentor to confirm and review the information submitted. This survey will be converted to a spreadsheet which I will use for recall purpose, to pass information and, most importantly, to get you commissioned into the Marine Corps. This document must be completed no later than 04 DEC 2015. I understand this information is privacy sensitive and will be treated as such; however, it is absolutely necessary in order to get you commissioned. If you would prefer to come see me personally to input the data, please stop by my office prior to the due date. You will receive this survey on 23 Nov 2015. Fill out the survey as soon as possible and click ‘submit’. Note that some of the information on this survey will be pre-entered and is not editable. If any of the pre-entered information is incorrect, you will need to let your Marine mentor know since this means that your data is entered incorrectly in the USNA database. Once you have completed the survey, check the box at the bottom of the page and hit submit. If you are not finished entering data but need to close the survey to gather information, simply hit submit without checking the box at the bottom (this will save your current progress). After you check the box at the bottom, make an appointment to sit down with your Marine mentor, at which time you will log in to your survey and review the data. Once your mentor is satisfied, they will write “Mentor review complete” in the mentor comments block at the bottom of the survey. Again, both requirements need to happen by 04 DEC 2015. In addition to the survey, you must complete several required personal documents listed below and provided as PDF/Word documents at the following link: http://www.usna.edu/USMC/Commissioning_Documents.php. All personal documents and PDF/Word documents are required to be complete and returned to me personally, no later than 15 JAN 2016. It is very important for you to review these documents and ask questions prior to your departure on Christmas break since many are likely located at your parent’s home. Furthermore, you should review all of the documents with your Marine mentor prior to final submission to my office. You will see two pre-commissioning meetings on the Master Calendar for 1/C Marine Selects. The first is on 04 DEC 2015 and the second is on 13 JAN 2016. I will send out details a few days prior to each meeting so that everyone has a reminder and the specifics. Expect 1250-1320 in Mitscher Auditorium for both meetings. Your commissioning packages are due to Headquarters Marine Corps (HQMC) on 1 Feb 2016. During these meetings I’ll answer questions and provide information to help you meet this timeline. A lot of personal data will need to be collected over Christmas break so pay very close attention to the requirements and ask questions if you have them. -Specific instructions for completing the initial survey are below. Some information will be auto-populated and the survey MUST BE COMPLETED AND REVIEWED BY YOUR MARINE MENTOR NLT 04 DEC 2015: - Confirm your full first name, last name and middle name. - Provide any suffixes (i.e. Jr, Sr, I, III, IV etc…) - Confirm your USNA Company. - Confirm your full SSN. - Confirm your date of birth. - Confirm your gender. - Confirm your component here (Ground, Pilot, or NFO). - Confirm your ASTB score. If you do not have a passing score and are a Marine Ground option who wishes to pursue an aviation contract, continue to take the ASTB until you obtain a passing score. Notify your Marine mentor of your intentions. They will, in turn, make a comment in the comments block of your survey and email me with your desire to convert to Marine Pilot or NFO. Update me when you have a passing ASTB score. If you have not passed the ASTB by the third attempt, I am able to submit a waiver for you only if you are one point short in one category. For example, the Marine passing score is 4, 6, 6, if you get a 4, 5, 6 and have attempted the test three times, you may qualify for a waiver. Keep ALL three ASTB score sheets since they are required to be submitted with your waiver. - Confirm your PFT run time, crunches, pull-up or FAH score. - Confirm your total PFT score. - Insert your ACT or SAT score. Official score sheets are required with your final package so confirm the USNA database number matches your official documentation. Directions on how to obtain your official scores are outlined below. - An official copy of your most recent SAT or ACT score MUST be submitted. You can obtain copies of your SAT from “CollegeBoard.org” and copies of your ACT from the ACT student web services at “actstudent.org”. - If you do not have a combined 1000 on the SAT or greater than a 22 on your ACT expect to require a waiver. - When you print out your score sheet from the official site, ensure your full name is somewhere on the print out. These are submitted to HQMC so simply having your first name on the sheet will not suffice. - Insert your personal phone number (e.g. the cell # you will have upon graduation so I can get in immediate contact with you). - Insert your personal email (not your USNA email) that you will have upon graduation so I can pass information to you. Please know that your USNA email account will be terminated shortly after graduation so a private email address required. - Your TBS Class request will be Delta, Echo or Fox. Those Marines applying for a Graduate Education program, list the TBS class you wish to attend should you be disapproved for the Graduate program. Those Marines planning to stay TAD after graduation pick Echo or Fox. If your TAD date is approved past Fox, your TBS date will be adjusted accordingly at a later date. Keep in mind a very small number of Marines will be authorized by the Senior Marine to remain TAD past Fox. Furthermore, the below number of USNA graduates have been assigned to each TBS class. It is important to understand that most Marine Selects will be attending Delta company. - Delta (28 Jun 16) – 155 USNA Marines (107 Ground / 47 Pilot / 1 NFO) - Echo (9 Aug 16) – 48 USNA Marines (16 Ground / 31 Pilot / 1 NFO) - Fox (13 Sep 16) – 51 USNA Marines (42 Ground / 11 Pilot / 1 NFO) - Delta company Marines will get graduation leave (free leave/non-chargeable) from 27 MAY 16 through 27 JUN 16. On 28 JUN 16, you will check directly into TBS and are not required to return to the USNA after graduation. The Delta company is scheduled to graduate on JAN 17. - Echo company Marines will get graduation leave (free leave/non-chargeable) from 27 MAY 16 through 27 JUN 16. On 28 JUN 16, you will check directly into 8th & I and return to the USNA for your assigned TAD. On 9 AUG 16, you will physically check out of 8th and I and report directly to TBS for Echo company. The Echo company graduation is scheduled for MAR 17. - Fox company Marines will get graduation leave (free leave/non-chargeable) from 27 MAY 16 through 27 JUN 16. On 28 JUN 16, you will check directly into 8th & I and return to USNA for your assigned TAD. On 13 SEP 16, you will physically check out of 8th and I and report to TBS for Fox company. The Fox company graduation is scheduled for MAR 17. - Those Marines going to VGEP, your leave will go from 27 MAY 16 up until two days prior to when you are required to report to your university. An extra day is needed so that you have time to report into 8th & I after graduation leave for admin processing and then to your university (example: 27 MAY – 06 JUN = graduation leave; 07 JUN = check in to 8th & I; 08 JUN = report to university). - Those Marines who plan on going through the IGEP or who will be TAD on the Yard are authorized to take 30 days of leave upon graduation; hence, your leave will go from 27 MAY 16 through 27 JUN 16. Your report date to 8th & I will be 27 JUN 16 followed by assignment to the Yard until TBS or you detach for the IGEP. Again, only a very small number of Marine selects will be authorized to TAD past Fox and no Marine select will be approved TAD past Fox until the TAD matrix is approved and disseminated in late February 16. For the purposes of this survey, all Marine selects should expect to attend Delta, Echo, or Fox. *** TAD – in order to not be assigned Delta, Echo or Fox company and have an extended stay here postgraduation, you must get approval through the Department with which you want to work and gain final approval from the Navy TAD coordinator and Senior Marine Corps Representative. Extended TADs will be by exception and will be approved for extraordinary reasons only (e.g. medical issues, extreme personal circumstances, by-name request by a Department requiring support to operate, etc). The total number of authorized TAD positions past Foxcompany is 27 Marines, this includes all categories above and cannot be exceeded. There are no graduation leave modifications authorized; therefore, those of you who will be doing competitions or who are asked by your TAD Department to check in from graduation leave early, know that you will forfeit any remaining graduation leave you do not use. The Marine Corps policy here differs from the Navy. You are required to check in to 8th and I 30 days after graduation; therefore, you have 30 days post-graduation to take leave, period. You also cannot check into 8th and I early, so if you decided to come back to the Yard to work early or to participate in an athletic event, you are doing so in a leave status. The bottom line is if you want to take the full 30 days of graduation leave, then you can and should. As always, you should let your TAD OIC know of your exact graduation leave dates and of the fact that the window is non-negotiable so that they can plan accordingly. If you get any push back or are promised graduation leave after you check in to 8th & I on 27 JUN 16, know now that this will not be authorized and is in direct violation of Marine Corps Order 1050.3J. Once you are checked in to 8th and I and start earning your full BAH entitlement you no longer rate free leave. I am the POC for any questions from your supervisors in this regard. - Your complete leave address. This is the location you will be at during your graduation leave (if you will be at multiple locations while on leave, put the address where you will be the longest or put your home of record/parents address). When typing in the address on this form, list the full address in one line. For example: 123 FAKE STREET, ANNAPOLIS, MD, 21401. - TAD location: For those of you requesting TAD upon graduation and those Marines applying for IGEP/VGEP, please put your preferred TAD location and end date of the TAD (e.g. PE Dept (Sprint Football) 07 SEP 2015). For the most part, your TAD will be somewhere here on the Yard. Keep in mind that Admissions and the PE Dept have the greatest need for assistance at the USNA, so the bulk of you will be working for them upon graduation. A by-name request to the Navy TAD coordinator is your best shot at receiving the TAD assignment you desire. Again, the TAD list will not be finalized until after Feb 15 so for the purposes of the survey simply put the location you desire now. Also, there are opportunities for a small number of you to work TAD off the Yard down in Quantico for example. These TAD positions involve HQMC (Marine Corps Recruiting Command or Marine Corps Information Operations Center). If you are interested in one of these two branches at HQMC then make note of that in this block. One caveat to any authorized TAD off the Yard is that you will rate transit BAH not full Annapolis BAH resulting in a loss of approximately $1,000 a month in your pay. BAH-T is what you will rate while you are on post-graduation leave ~650/mo. Once you check into 8th & I or TBS you will be given the local full BAH rate ~1650/mo for Annapolis. I will send all the pay and allowance information in a separate email as commissioning nears. - Your emergency contact is the name of your Primary Next of Kin (NOK) and their relation to you (e.g. mom, dad, etc, NOT your boyfriend or girlfriend). - For your emergency contact, I need their phone number should something happen to you. - HOR: Stands for Home of Record, where you officially call home for tax purposes, voter registration etc. This has been an issue in the past with 2ndLts trying to change their HOR post-commissioning because they realize they could have used a state tax free state. You will not be able to change this once it is submitted to HQMC unless you provide substantial proof. If you have a justification for listing a tax free state, e.g. FL or TX then this is the time to do so. Do not ask me to adjust this after you are processed by HQMC unless you have tax documents, proof of residency or a voter registration card from the state you want to switch to. You should list the state that you call home or are registered to vote in here; that said there is no proof required for the initial HOR request only for changes after processing by HQMC. Address must be listed in one line of data. For example: 123 FAKE STREET, ANNAPOLIS, MD, 21401. - HOR Country, State, City, County, and Zip. - Country of Birth: Enter country of birth i.e. USA or MEXICO. - Place of Birth. Provide city and state in the format: DALLAS, TX. - Citizenship: Read the codes in the column to determine and select the appropriate option. - Race and ethnicity: Refer to the Race category document and select the appropriate response. If none of the options match up with your race or you are a combination of different races, select the one that matches your race best or select ‘declined to respond’. Select whichever option you are most comfortable with as that is how you will be categorized in the future. - For example, if you are Mexican, you can put either White for race and then Mexican for ethnicity or select ‘declined to respond’ and then choose Mexican for ethnicity. - Another example, you are a mix of White and Black or Black and Asian, pick the race that you feel best describes you then for ethnicity you could choose the other half (e.g. Black (European/Anglo) or White (Korean), etc). - Religion: Click on the box and choose the applicable option. - Tattoos: Indicate how many you have (review the Marine Corps tattoo policy to determine if a cluster or sleeve, depending on size, constitutes one tattoo or multiple tattoos). If you have questions ask your Marine mentor or come by my office. - Plan on getting married prior to TBS? If you plan on getting married prior to TBS, indicate it here by stating “YES”. This is required since TBS needs to know in order to ensure suitable billeting is available for each TBS class. - Indicate whether or not you are applying to a graduate education program (IGEP, VGEP, or deferred graduation education scholarship). - Prior Service: If you have prior service, indicate it here by the following: - If you went to NAPS prior to the USNA but did not have prior service, just put “NAPS.” - If you are a prior service member and you went to NAPS indicate which branch you served in. - If you did not go to NAPS and are not a prior, just state “NONE.” - Awards/honorary letters: If you have received any by name, personal, awards since you have been a student at the USNA or if you were prior enlisted and received a personal award that is not on your DD214 (e.g. Navy Achievement Medal, Certificate of Commendation) or honorary by name letters while at the USNA (e.g. Special Recognition for civilian volunteer work, etc); identify it here by stating how many. Do not put any awards such as Varsity letters, athletic certificates or academic certificates. ** Again, you may revisit the survey at any time before 4 Dec and save your progress by hitting the submit button. When you are ready to review the survey with your Marine mentor, simply check the box at the bottom, hit submit, and email your mentor to set up an appointment. Once you have reviewed the information with your mentor, they will write “Mentor review complete” in the comments block and hit submit a final time. - Required personal information and specific instructions for completing the PDF/Word documents are below. All documents and supporting documents are located at the following link http://www.usna.edu/USMC/Commissioning_Documents.php. All personal information and PDF/Word documents MUST BE COMPLETED BY 15 JAN 2016 and turned into me, in person (Sampson Hall, Room 265). When you turn the forms in, have at least 30 minutes available to go over the documents and the information you provided in the survey. Since I need to review over 271 packages, do not wait until the last minute to come see me. Once you have everything together come to my office. To help facilitate the process, ask your Marine mentor to sign anywhere it states Officer Signature, Recruiter or Witness. If a form doesn’t apply to you, simply put N/A at the top and complete the required data at the bottom of the page. For example, if you have never experimented with drugs, you will write N/A at the top of the Drug Statement Form, fill out the bottom section, sign/date and have your mentor witness. Also ensure you scroll down and list your program as “USNA” NOT NROTC on all documents where it applies. - Provide a copy of your Birth Certificate (no originals, you can use the copy machine next to my office if needed). If you don’t have one, get with the Midshipmen Personnel Office or the Admissions Office at the USNA to see if they have one on file. If you still cannot find a copy, you will need to contact the county/city or hospital where you were born and get a copy through them. This is a requirement to commission you; therefore, not being able to locate it is not an option. Make sure the copy you submit is legible. - Provide a copy of your SSN card (no originals). If you don’t have one and your parents don’t have a copy, do NOT waste time looking all over your storage at your parent’s house for it. Go to the Social Security website (http://www.ssa.gov/) and request a new one ASAP. The website has instructions on how to get a new one, and it typically only takes 10 business days. - For prior service personnel, provide me a copy of your DD-214. This very important, without your DD214 your prior service will not be credited. For those who went to NAPS, get your DD-214 from the Midshipmen Personnel Office and submit it with your package. - For prior Marines, if you do not have a copy of your DD-214 you need to either go to MCB Quantico, VA building 2008 Elliot Road, during normal working hours (Mon – Fri) to obtain one (have with you a photo ID) or you need to call 1-800-268-3710 and request a signed copy. - For prior service personnel of other services (Army, Navy, or Air Force) you need to contact the records branch at your respective service in order to get a signed copy of your DD-214. - Provide a copy of any Awards/honorary letters not listed on your DD-214. If you are in doubt, make a copy of it and come talk to me about it. I’ll submit this copy with your commissioning package; however, your awards/honorary letters cannot be submitted into your official file until after you are commissioned into the system. Your Marine profiles will be officially set up after you check into either 8th & I or TBS so expect your awards to be submitted to HQMC then. Always keep copies for your records. - If you are going to TBS for Echo company, you will turn in a copy of your Awards/honorary letters to the admin section upon check in at TBS so they can incorporate them into your record. - Those Marines going to Fox company or a later TBS class, you will come see me with additional copies of your awards upon your return from leave and I will assist in incorporating everything into your personal record. Again, anything on your DD-214 will be added to your commissioning papers so additional action postgraduation is not required. - Print out the PDF/Word documents at the following link: http://www.usna.edu/USMC/Commissioning_Documents.php and complete all requirements where indicated. Have your Marine Mentor sign where it says Witness, Marine Corps Representative, Recruiter or Commissioned Officer. If the form doesn’t apply to you, print it out and write “N/A” at the top so there is a record that you have read the document. If you cannot type the information into the form, clearly print your name anywhere it says “type.” Once ALL of the documents are complete and you have gathered all of the personal information required, place them in the order preference provided, make an appointment to conduct a final review with your Marine mentor, and then bring them to Sampson 265 for audit and submission to HQMC, again NLT 15 Jan 16. - Request for Appointment (RFA) to Commission form, complete this form in its entirety. A common mistake is leaving MIDN Joseph David Marine in the “From” line. The example form provided is to help you complete the form. Know that the “From” line is where you insert your full name, full SSN and your MOS. If you are Ground its 8001, Pilot is 7599, and NFO is 7578. If you are looking to change from Ground to Pilot or NFO you are still Ground at the time of this form so put 8001. If you are later approved for Pilot or NFO we will update your MOS then. Below will help you with the rest of the form. Please make sure you unhighlight the example RFA prior to submission. - 3. Put and X before Service Academy - 4. Put an X before your current component - 5. Put an X where appropriate - 6. Scroll down and insert the appropriate number and letters per the example - 7. Type in your desired TBS date and the date it convenes. This will likely change to meeting the TBS requirements but list what you prefer here. - Sign and have your Marine mentor witness - For the MCRC Officer Tattoo Screening Form, complete your information at the top of pages 1 and 2. Answer the questions in part I, II, III, and IV and V. If you do not have tattoos, you may skip the “note” portion of part II and all of part IV only. If you indicate you have a tattoo, list it in part IV and use the addendum pages to describe each tattoo via the directions below. One tattoo per addendum page please. You will be contacted in January 15 to set up an appointment for photos. Once an appointment is scheduled, arrive in your USNA PT gear (shorts, t-shirt, socks and running shoes). If you have visible upper body tattoos you will be required to take a photo in a Marine Service “C” shirt (khaki short sleeve). On the addendum pages you will need to provide a detailed description of each tattoo. At a minimum this needs to include: - Date you got it (month/year). - Dimensions in inches (length and width). - Location of tattoo (where it is precisely on your body). - What it is and says. - What it means to you (in other words, why did you get it). ***If the tattoo is in a private area (not visible in t-shirt and shorts for females or in shorts for Males), you will either draw the tattoo freehand on the addendum page and provide the details above, or download a drawing of it from the Internet and include your description on the printed page. DO NOT take an up close photo of it and give it to me! - Tattoo Statement of Understanding: Print your name, sign and date. Have your Marine mentor complete block 4 and 5. - The Record of Emergency Data (RED) and Service Members Group Life Insurance (SGLI) forms are temporary, meaning once you check into either 8th & I or TBS you will complete this again. The reason I am having you do it now is just in case something happens to you while you are on graduation leave, prior to checking into 8th & I or TBS. Specific instructions to complete these forms are as follows: - For the RED: - Complete blocks 1 - 17. - Leave block 10 blank. - For Block 11 - 13, should you die while on leave, whomever you designate in this block will receive $10,000 to cover your funeral cost, receive any unpaid pay and allowances and work directly with a Marne Corps Official in order to determine your place of burial and other associated factors. I recommend choosing a parent(s) for this block or someone who you are extremely close with and have been close with all your life. - For Block 14, if you live in a remote area and the only way to find your place is by turning right at the crooked tree, put directions in here. Also, if you have any other extenuating circumstances that someone should know upon letting your NOK know of your death, advise here (e.g. parents only speak Spanish, mom has a heart condition, dad carries a gun, etc). - For SGLI, no mistakes, scratch outs or errors of any kind are allowed and you must print or type legibly; hence, if you make a mistake, print off a new one and do it again. - Complete the top portion where it requests your last name through SSN. For the rank, put in “2ndLt” (note that your first rank is not spelled 2LT or LT). - Skip to the Principal and Contingent blocks. Once you check in at 8th & I or TBS you can elect to have less than $400,000; however, for the month that you are on graduation leave, you all will be getting $400,000 in coverage as you are going to wind up paying for it regardless of the coverage you prefer (I am not capable of getting you a refund). - For Principal select who you want to get the $400,000 should you die (I recommend choosing a parent(s) for this block or someone who you are extremely close with and have been close with all your life). If you want to break it up between parents or siblings you can do so; however, keep it to no more than 3 people. Once again, once you check into 8th & I or TBS you will redo the form and can designate more people if you like. - The Contingent portion is for just in case something happens to whomever you designated in the Principal (e.g. mom and dad who were your principals are in the same car as you when it crashes and you all pass away, then the contingent comes into play, meaning DON’T put mom as the principal and dad as the contingent if they are living together). The bottom line is you need to name at least one contingent (who is not a principal) and provide their information. - Sign and date and have your Marine mentor witness it. - Print off the PDF titled “Direct Deposit Form:” - Take this form directly to your financial institution and they will complete it for you. - If you are going to bank with USAA, they have an 800 number you can call (Google it to get the number). Make sure the bank completes the bottom potion of the form where it states representative. HQMC will not process this form without it being completed. Significant and long lasting pay issues will exist if this is not done correctly. DO NOT put your USAA acct number and your Navy Federal routing number or vice versa. Play very close attention to the accuracy of this form since it is the main document between you and getting paid on time. I’ve included a USAA direct deposit form under the document link for those of you who bank with USAA. - Just in case there are questions, the only blocks that need to be completed are: - Block A – Need full name, address and phone number (use the address that your bank currently has on file) - Block C – Need full SSN here - Block D – Mark if checking or savings - Block E – Need your account # - Block F – Check the block “Mil Active” and write USMC on the line - Sign and date under “Payee/joint payee certification” - Ensure your financial institution completes Section 3 in its entirety - Drug Statement Form: Type and sign your name at the bottom, input the date and your full SSN. Select the drop down arrow and select “USNA” for the program. Have your Marine mentor witness. - USMC Drug Policy Statement of Understanding: Type and sign your name, input date and your last four. Have your Marine mentor sign under Officer signature, date, and print their name. - Civil Relief Act Statement of Understand: Type your name at the top. Sign and date at the bottom of the page. Have your Marine mentor sign and date at the bottom. - Fraternization Statement of Understanding: Type your name under participant and at the bottom. Sign, date, and type in your last four. From the drop down select “USNA” for the program. Have your Marine mentor sign and date. - Restrictions on Personal Conduct Statement of Understanding: Type and sign your name and input the date. Have your Marine mentor type their name and sign. - Sexual Assault and Harassment Statement of Understanding: Type and sign your name, input the date and the last four of your SSN. Have your Marine mentor type their name and sign where it says “Recruiter Verification”. Number 5 can remain blank. - Privacy Act Form NAVMC 11000: Sign, input the date and full SSN. - Non-traffic Arrest Form: Fill out only if it applies. If it does not apply to you write “N/A” at the top and turn it into me with the rest of the documents. Ensure you type in your full name, full SSN and the date. Sign at the bottom and have your Marine mentor witness. Print out additional pages if you have more than one non-traffic arrest and use an addendum page if you need more room on a single incident. Non-traffic arrests include, drunk in public citations, shoplifting, assault, etc. Your statement needs to be in detail outlining when the event took place, what happened, what you were cited for originally, what it was reduced to, the fine you paid, and any extenuating circumstances. Make sure you sign and date at the bottom of any addendum pages or statements. - All non-traffic related arrests must be declared and documented. For instance, you were arrested or cited for assault, you will be required to declare this event on the form, write and sign a statement outlining the details and submit copies of any tickets received and proof of payment. I will generate the waiver and let you know when it is approved. - If you received probation or anything other than or in addition to a monetary fine, you will need to provide proof that you have satisfied that requirement in full. - Minor Traffic Page: Complete as required or write “N/A” at the top. Type and sign your name, correct the date, input your full SSN and select “USNA” for the program from the drop down bar. Have your Marine mentor sign. Read the top position of the document carefully and comply in each block. If you mess up, start over so that all details are legible. - If you have multiple tickets within the last five years that amount to over $500.00 total AND/OR you have a single ticket in the last five years that resulted in a fine greater than $250.00, you must provide a copy of each ticket and proof that you paid it. The best way to do this is to request a copy of your driving record which should show your violations and the fact that they are paid in full. If you know you fall into one or both of these categories but cannot find any documentation on the tickets, again you must call the court house or DMV and supply driving record which shows that you have no outstanding judgments against you. This takes time so get on the requirement early if it applies. If you have the tickets and proof of payment simply complete the form and submit the documentation. If you have multiple tickets that amount to less than $500.00 or less and no single ticket with a fine greater than $250.00 simply complete the form and submit it with your package. For example, I have four tickets, two are small parking violations and two are small speeding tickets. The total fines for all tickets amount to $450.00 = no proof required, simple fill out the details for each ticket in all four blocks and sign, date, witness. If I have six tickets and the total fines amount to $501.00, I need to supply copies of all the tickets and proof of payment or a police record check that shows the violations and proves that the fines are paid in full. Lastly, if I have one ticket and the fine was $300.00, I need to supply a copy of the ticket and proof of payment or a police record check that shows the violation and proves all fines are paid in full. If you have questions, ask. - If you received probation or anything other than or in addition to a monetary fine, you will need to provide proof that you have satisfied that requirement in full. - USNA Waiver Information Sheet: Fill out the form in its entirety. Full name, SSN, and 22 May 15 commissioning date across the top. Read and answer the questions. If you answer “yes” to any question, provide a written statement, on a separate piece of paper, and submit it with your package. Sign and date at the bottom and have your Marine mentor sign and date. If you write a statement, use the addendum page provided and sign and date after your statement. Write clearly and provide details. - Print off the applicable contract which you have been assigned: - Ground contracts: Print out the PDF titled “Ground Contract,” read and complete the information therein. Print, sign, and date at the bottom and have your Marine mentor witness. - Air contracts: Print out the PDF titled “Air Contract,” read and complete the information therein. Print, sign, and date at the bottom and have your Marine mentor witness. - NFO contract: Print out the PDF titled “NFO Contract” AND the PDF titled “Air Contract,” read and complete the information therein for both. Print, sign, and date at the bottom and have your Marine mentor witness. - If you are pending a contract switch fill out only the contract you are currently approved for. For Ground Marines who later pass the ASTB and are approved to switch to Pilot or NFO, a new contract will be signed and the existing one will be shredded. A few final notes: - If you have a medical issue or appointment for your commissioning physical, get to medical ASAP. You must be cleared medically for commission prior to graduation. If you are not medically cleared it will be very difficult, if not impossible, to commission you with the rest of your classmates. Bottom line, stay engaged with medical until you are cleared and let me know if you hit any snags. - For those of you going Pilot or NFO, get to medical and get your flight screening started as soon as you are told to do so. If this is not complete and you do not have medical clearance from NAMI, you stand the chance of losing your Pilot or NFO contract come graduation. To recap1.
