The purpose of this residency program is to prepare pharmacists who are passionate about public health to acquire the skills necessary to operate a community pharmacy and to develop Clinical Pharmacy Services in a primary care setting. The Program Design: Collaborating to provide care for the community Pacific University School of Pharmacy offers a three-year, year round curriculum leading to the PharmD degree. The school is housed in the Health Professions Campus along with seven other health professional programs. The resident will complete the Oregon Pharmacy Teaching Certificate Program and will teach alongside faculty in the curriculum. The resident will participate in student learning on pharmacy practice days and will facilitate learning activities for students on APPE rotations. The resident may also participate in Pacific patient care clinics and interview days. Virginia Garcia Memorial Health Center (VGMHC) Hillsboro Clinic is a community health center that is situated on the ground floor of the Health Professions Campus. The resident will have the opportunity to train with a well established Clinical Pharmacy Services team that works directly with patients and medical providers to maximize medication therapy under collaborative drug therapy management protocols. In addition to rotations in the clinic, the resident will train in the onsite 340B pharmacy to develop skills in operating and managing a pharmacy in a community health center. Tuality Healthcare Located across the street from the Pacific Health Professions Campus, Tuality Community Hospital provides care to many VGMHC patients. The resident’s role in training at Tuality is to gain a complete understanding of how patients receive care across the continuum. The resident will complete orientation at Tuality, then will spend time each morning working with hospital pharmacists and staff to identify VGMHC patients and ensure accurate medication reconciliation at admit, participate in discharge planning, and coordinate follow-up visits in the primary care clinic.
