Purpose:
Toassurethatthebasicrightsofhumanbeingsforindependenceofexpression,decisionandaction,
concernforpersonaldignityandhumanrelationshipsarepreservedforallpatients,andtodefinethe
responsibilitiesofpatientsseenatKirkwoodCampusHealth.
Policy:
ItisthepolicyofKirkwoodCampusHealthtorespecttheindividualrightsofallpersonsthatcometo
thisCampusHealthforcare.Patientsincludetherighttomakedecisionsregardingmedicalcare,the
righttoacceptorrefusetreatment.Patientresponsibilitiesincludeactionsonthepartofpatientsthat
areneededsothathealthcareproviderscanprovideappropriatecaremakeaccurateresponsiblecare
decisions,addresspatients’needs,andmaintainasoundandviableCampusHealth

A. AccesstoCare
Individualsshallbegivenimpartialaccesstotreatmentthatisavailableandmedicallyindicated,
regardlessofrace,creed,sex,nationalorigin,religionorsexualorientation.
B. RespectandDignity
Thepatienthastherighttoconsiderate,respectfulcareatalltimes,underallcircumstances,
withrecognitionofhispersonaldignityandworth.
C. PrivacyandConfidentiality
Thepatienthasarighttopersonalprivacywhileinourclinicandtohaveallinformation
regardingillnessesandcarestreatedconfidential.
D. PersonalSafetyandSecurity
ThepatienthastherighttoexpectreasonablesafetyinsofarastheCampusHealthpractices
andenvironmentareconcerned
E. Identity
Thepatienthastherighttoknowtheidentityandprofessionalstatusofindividualsproviding
service.
F. Information
Thepatienthasarighttoconfidentialtreatmentofdisclosuresandrecordsandanopportunity
toapproveorrefusetheirreleaseasdescribedintheKirkwoodCommunityCollegeCampus
HealthCarePrivacyNotice.
G. Consent
Thepatienthastherighttomakeinformeddecisionsregardingcareincludingtherighttoaccept
orrefusemedicalcareortreatment.
H. Concerns
YouhavearighttoexpressconcernsorcomplaintsregardingyourcaretoKCCadministration
andparticipateinaformalgrievanceprocedure.

healthcareprofessionalandclinicstaff.
B. Respectclinicpolicies.
C. Keepingyourscheduledappointmentsorcancelingyourappointmentsasfarinadvanceas
possible,sothatthetimemaybegiventosomeoneelse.
D. Providingaccurateandcompleteinformationaboutallmatterspertainingtoyourhealth,
includingmedications,allergies,pastorpresentmedicalproblems,perceivedrisksandwishes
foryourmedicalcareaswellasanyotherinformationyouprovideCampusHealth.
E. Seekingclarificationifyoudonotfullyunderstandyourhealthproblemandtheproposedplan
ofcare.
F. Followingthroughonyouragreedplanofcareordiscusswithyourproviderreasonsfornot
followingtheplan.Acceptingtheconsequencesofyourdecisionifyouchoosenottofollow
