 Introduction – provide an overview/background of the co-op experience
(when, where, company/site details, information about the opportunity etc.)
 Projects – what projects did you work on individually or in a team setting
 Reflection – what did you learn, how did you change, new skills formed,
current skills enhanced etc.
 Future plans – how has this Co-op impacted your college career and your
future careers goals
 Any other pertinent information to support your self-nomination
Section III: Updated Resume
Directions: Submit a high quality resume which includes your recent Co-op position
Section IV: Unofficial Transcripts
Directions: Obtain through Access Stout
Section V: Letter of Recommendation
Directions: Please send the on-line recommendation form (link below) to obtain one
recommendation for this award. An appropriate recommendation could be from your
Co-op Site Supervisor, Upper Management at Co-op site, Co-op Faculty Mentor, UWStout faculty, UW- Stout staff etc. Remember to request letters of recommendation at
least one week in advance.
