REPORT WRITING DIRECTIONS
UW-Stout Career Services
On each report you submit, please include the following:
• Your Name
• Employer Name
• UW-Stout Faculty Mentor Name
• Co-op Term (start & end dates)
• Date of Report
• Number of Report (1 of 3, 2 of 5, etc.)
INTERIM REPORTS - Respond to each of the following six statements for all reports except the final report.
1. List your job duties. Be specific.
2. Are you experiencing a relevant and challenging use of your abilities? Explain in terms of training received,
responsibilities given, and your professional growth.
3. Describe progress being made in reaching your objectives. Address objectives and achievements separately. If
objectives have been modified, explain.
4. How do you feel your experience has helped you to gain a better understanding of your area of interest?
5. Are you satisfied with the communication and contact, which you have with your on-the-job supervisor? If not, what
steps are you planning and taking to improve your on-the-job communication/supervisory contact?
6. Is there any way in which your faculty mentor or the co-op office can be of additional help to you?
The FINAL REPORT should be a comprehensive analysis of your learning covering the entire co-op. It should
consist of a title page, a table of contents, and the main body. The final report should be 6-10 pages, typed,
grammatically correct, and very well done. The following issues should be addressed:
1. A general history of the organization.
2. The relationship of your position to the organization as a whole.
3. The regular duties you were expected to perform.
4. The relationship of your co-op to the courses you have taken at UW-Stout. Also, discuss courses you plan to take in
the future that would better prepare you for your career.
5. What you learned about yourself as a result of this co-op.
6. What type of career is now more attractive to you as a result of this experience?

For your protection, keep a copy of each report until you receive a final letter

Reports are to be submitted via the web at:
To submit your reports please log onto your Profile at: www.uwstout.edu/careers, choose Students-Stout Careerlink, log
on, (your User Name is your Stout ID, usually without the preceding 0’s and stout e.g. 216485stout and your Password
(unless you’ve changed it) is the last 4 digits of your Stout Id e.g. 6485. Then, choose My Documents which is located on
a scroll down menu under My Account. Scroll down to Co-op Reports and select Upload File. Select the file to upload,
name this document and submit. It is recommended that file names’ be submitted with report number and your name in
the Document Name field. Example: Report 1 – James Stout. If you have problems logging on, please contact us and we
will help you!
After submitting your Co-op report, your Faculty mentor and the Co-op Email box will automatically be sent a message
stating that your report has been uploaded. This system allows all paperwork activity to be electronically tracked.
Please note that you will always be able to log onto your Profile and see your reports.
When uploading reports please take the time to update your Co-op information; such as your address, phone number, etc.
