Bacteriological Monitoring Schedule, Worksheet, and Summary. Report 160, Bacteriological Monitoring Schedule, Worksheet, and Summary (Exhibit 1) is used to record microbiological monitoring and followup actions taken at potable water systems. When kept up to date, the report serves as a management and program review document, as a recordkeeping device and satisfies various Federal, State, and Manual requirements. The purpose of the entire microbiological monitoring program is to insure microbiologically safe water. The forms and charts serve only as a tool to meet this objective. The major key to meeting this objective is timely and effective response to correct a water system deficiency when the microbiological indicator suggests a problem. Action to be taken is defined in FSM 7421.26 - Followup Action and in FSM 7420 Table 1.-- Allowable Microbiological Contaminant Levels and Followup Action. Blank forms will be provided prior to the beginning of the new calendar year. 2. Instructions for Completing Report 160, Bacteriological Monitoring Schedule, Worksheet, and Summary. The purpose of this form is to record the microbiological monitoring and followup actions for one calendar year. Use pen or pencil, typing is not required. Review Exhibit 1 for general layout and information entry. The use code must be filled in. It may or may not match the identifier code in the water system name. For seasonal systems, record the month and date of the first negative test taken before the system is opened to use. Enter the month and day the system was opened and closed to use. For systems open all year, use 1/1 and 12/31 as opening and closing dates. All routine tests are recorded as dates. Circle the date of each routine positive test. A positive test is either R6 SUPPLEMENT 7409.11-88-4 EFFECTIVE 02/88 7409.11,90 Page 2 of 4 a membrane filter test with one or more colonies including confluent growth or multiple tube test with one or more positive tubes. A positive test requires an entry in the followup action portion of the form. Consult Table 1.--Allowable Microbiological Contaminant Levels and Followup Action in FSM 7420 for appropriate action codes and resample actions. Record the resample results and action taken as appropriate. Positive test results must be entered under "MF" for membrane filter or "MT" for multiple tube technique.
