Note: The director of graduate studies must be notified using this form ten business days
in advance of the examination. Once this form is received, department faculty and
graduate students will be notified by memo or email. In addition to submitting this form,
a copy of the M.A. thesis, the Ph.D. proposal, or the Ph.D. dissertation must be delivered
to the main department office ten business days before the exam for faculty review.
