Job Description
POSITION TITLE:
Mental Health Specialist /Community Support Worker
PROGRAM:
Mental Health Rehabilitation Cognitive Institute Inc.
REPORTS TO:
Licensed Mental Health Professional, Mental Health Professional, Program Director, Executive
Director
SUPERVISED BY:
Licensed Mental Health Professional, Mental Health Professional, Psychiatrist
RESPONSIBILITIES:
1. Provide skills training, service integration, and clinical management coordination.
2. Assist in developing and implementing plans of services and/or social work treatment.
3. Consult with physician, nurses and other clinic personnel and community agency personnel to develop social
diagnosis and plan of treatment or service and to interpret psycho-social factors related to illness, treatment and
continuity of care needs.
4. Provide assistance in an interdisciplinary or multidisciplinary team approach to diagnosis, plan, and provide
treatment.
5. Assist in arranging for and coordination of delivery of services within the hospital and in the broader community
to address continuity of care needs.
6. Document services provided in accordance with clinical standards.
7. Participate as a member of staff, work unit and/or department.
8. Committee in programs and resource activities.
9. Communicate needs and/or problems with LMHP(s) to allow for rapid problem-solving.
10. Meet monthly with LMHP(s) for supervision.
11. Provide business office with all required documents for personnel records.
12. Enter progress notes in Clinical Advisor within 24 hours of seeing recipient with proper documentation and
appropriate signatures.
13. Print all completed progress notes weekly.
14. Enter recipient’s schedule in Clinical Advisor weekly.
15. Regulate proper usage of Prior Authorization units for all recipients.
16. Provide leadership and/or assistance in therapeutic groups.
17. Attend all staff meetings.
18. Attend meetings and/or trainings that will benefit Cognitive Institute Inc. as requested by supervisor. (i.e.
Magellan, community involvement, etc.)
19. Return phone calls promptly.
20. Follow all safety rules.
21. Obtain 40 training hours as required for a full time CSW or 20 training hours as required for a part time CSW
yearly.
22. Actively participate in all evaluations conducted by Cognitive Institute Inc.
23. Actively participate in crisis interventions.
Minimum Qualifications:

A Bachelor’s degree in a mental health related field;
•

or
A Bachelor’s degree enrolled in college and pursuing a graduate degree in a mental health related field and has
completed at least two (2) courses identified in a mental health related field;
or
A high school diploma or a GED, and at least four (4) years of experience providing direct services in a mental
health, physical health, social services, education or correctional setting.
NOTE: The MHS must work under the guidance of an LMHP.
