MENTOR TEACHER AGREEMENT
Thank you for agreeing to serve as a mentor teacher for Apprentice Teaching (AT) this semester! This document
outlines the duties and expectations of a mentor teacher. Please review these expectations carefully and sign the
agreement at the bottom of the second page. Contact a course instructor if you have any questions or concerns at
any time.

As a Mentor Teacher, I will:
Supervise UCCSTeach teacher candidate for a full semester
Meet with the UCCSTeach as needed
Provide opportunities for UCCSTeach teacher candidates to develop their skills in the classroom
Aid the UCCSTeach teacher candidate in the development of their Electronic Portfolio and edTPA
Complete all required paperwork (observation forms, disposition forms, activity forms, etc.)
Use email on a regular basis to communicate effectively with the UCCSTeach staff
Provide effective and consistent feedback to the UCCSTeach teacher candidates

Stipend
For your contribution to the growth of these future teachers, you will receive a $250 stipend and a onetime $50 stipend for attending the Mentor Teacher Training meeting will be paid in addition to your
mentoring stipend. Your check will be mailed to you upon completion and receipt of all evaluations. This
will include one lesson feedback form per team per lesson, plus one final evaluation per student.
