To ensure we are in full compliance with the Construction (Design and Management) Regulations 2015 as well as our
own ISO quality, environmental and safety management procedures we require that all our sub-contractors complete
the attached SQ1 – Sub-Contractor Questionnaire.
To enable the questionnaire to be approved please complete all sections to the best of your ability, providing as much
detail as possible. This will enable the originator of the SQ1 to approve your company without delay.
Questions relating to the completion of the form should be directed to the originator. If you need specific information
on membership to receive safety/environmental competent advice or inspection services (Question 14) you can also
contact The Building Safety Group (BSG). If you need company training (Question 15) you can also contact
Bedfordshire and Hertfordshire Construction Training Limited (BHCTL).
Both BSG and BHCTL are not for profit groups who work for the benefit of their members striving to improve our industry

Policy includes all listed items or statement is included verifying they will comply, as a minimum, with the requirements
contained within our Health and Safety Policy and Procedures. Where there is doubt as to the adequacy of the H&S policy
for the type of works the contractor is to undertake the HSQE Manager is to be consulted.
14:
Should supply evidence of qualifications or experience to carry out this role and may include IOSH, NEBOSH, CITB, CSCS
or other relevant qualifications (note that some companies may use external consultants, in-house operatives or directors to
fulfil this role). Where industry experience is the only evidence HSQE should be consulted prior to approval.
15:
Training records should include H&S records for Management/Supervisory Staff as well as applicable operative training e.g.
asbestos awareness, plant operators, working at height etc and may include on the job, in-house or external training. The
HSQE Manager is to be consulted where recent training, relevant to the works to be carried out cannot be provided.

The contractor has over 90% of their on site workforce qualified to CSCS or provided a commitment to do so within 3 years.
Where accidents, dangerous occurrences, HSE prosecutions/prohibitions have been identified the contractor should identify
actions taken including actions to prevent recurrence of a similar incident.

The HSQE manager is to be consulted where the contractor has had any Fatalities, Major Accidents, Dangerous
Occurrences or Reportable Diseases or if the AIR is over 1000 prior to approval.
