COMMUNITY SUPPORT COORDINATOR

Job Title:
Community Support Co-ordinator (CSC)
Salary:
£15,500
Annual Leave for this post: 30 days per annum
Reporting Line: Support Advisor; people who we work for, their family or circle
Disclosure: The successful candidate will be required to undertake an application to confirm their
suitability to work with vulnerable adults and children under the protecting Vulnerable Groups
(PVG) Scheme which discloses spent and unspent convictions.
Purpose of the Role
The Community Support Co-ordinator will work with up to 3 people that choose to be supported by
Inclusion. You will work closely with Families and Circles of Friends, not only in strengthening
relationships but also in the development of quality services and in ensuring the experiences of
people receiving support from Inclusion are positive and meeting their needs and aspirations.
You will be responsible for managing the teams supporting the people we work for and
co-ordinating the unpaid supporters they have in their lives; identifying possible Circle members,
ensuring that each person’s outcome based support plan is being implemented and is progressing
satisfactorily.
You should be non judgemental, highly motivated and committed to the values of Inclusion. This role
demands flexibility, confidence and resilience. You should have a positive “can do” attitude, you
should be creative and a strong team player.

Key tasks
Co-ordinating the support for people who choose support from Inclusion at all times using person
centred approaches, you will be responsible for the implementation of the outcome based
support plans and ensuring support needs are met in line with the plans.










Co-ordinate the implementation of the outcome based support plans for up to 3 people
who use our services
Develop strong community links, community building and mapping opportunities for people
to be connected and to benefit from local community resources
Develop a positive working relationship with people who we work for their families and
circles to ensure effective supports are established to meet their needs and expectations
Co-ordinate and ensure that the Individual Service Funds( individual budgets) and spending
plans are within budget
Assist those supporting the person we work for maximise their income and make best use of
the resources available to them
Foster and develop relationships to form Circles of Friends
Support and co-ordinate Circles of Friends meetings for people we support and help people
maintain their circle of friends and family relationships
Working alongside people we support and their families or circles, supporting them to be
more in control of the running of their service.
Support the people we work for to be more connected in their community,
Support them to be good neighbours and to build relationships within their local area by
supporting the people we work for to access local amenities such as libraries, parks local
shops.
Organisational key tasks









To work within and adhere to Inclusion’s Policies and Procedures
Co-ordinate and administer the support arrangements for the person we work for including
both formal and informal supports
Implement support arrangements as detailed in the Contract
To ensure all medical records and financial transactions, accidents and incidents, meet the
organisational procedures and the requirements of Care Inspectorate and are lodged
centrally timeously.
To ensure accurate records are maintained for all the day to day expenditures and
communications within the homes and that there is a complete and accurate record of
people’s achievements and plans
Meet with families or Circles to help people develop and maintain good relations and to
ensure avenues of good communication are maintained as detailed in the support contract.
Co-ordinate and take part in person centred planning and in the annual outcome based
support plans with the person we work for and their family and circles
Be the first port of call for support and advice to the person you work for , their family or
circle and their staff
Maintain confidentiality at all times

In monitoring the quality of the support gather evidence from the person we work for, their
Family or Circle, care manager and any other professional or persons important in their lives
to collate a report. This in turn can inform the Board of Trustees on a bi monthly basis of
achievements or potential issues
Take responsibility for own personal development and training ensuring that all core training
is up to date and person specific training is met
Co-ordinate the training needs for the teams that you work with and provide coaching and
mentoring for the staff within the teams.
To attend regular peer support sessions on a regular basis
To carry out any reasonable other duties as required by the Organisation




Skills Experience and Knowledge
Essential
















Management or Supervision experience essential, with experience in a similar role
preferred
Full Clean Driver’s Licence
Ability to Plan effectively and co-ordinate a range of activities and events
Excellent communication skills ( written and oral)
Excellent listening skills
Excellent inter personal skills
Competent computer skills.
An approachable and friendly manner
Ability to work as a team member
Ability to co-ordinate and develop paid and unpaid supports
The ability to think creatively and come up with imaginative solutions
Excellent time management skills and the ability to prioritise and organise
It is also important that you are a good community builder and connector
You should be highly motivated and be able to motivate others
You should have perseverance and resilience
This post will be registered with the Care Inspectorate and the post holder should have,
or be working towards an appropriate health and social care qualification and
management qualification at level 3 or equivalent professional qualification accepted by
the Care Inspectorate.
