JOB NUMBER: JOB TITLE: Nurse Practitioner Part-time Contracted – Grand Rapids Office Supervisor: Clinical Manager Status: Exempt Salary: Grade 11 POSITION SUMMARY The clinician will report to the Clinical Manager. The person in this position is licensed to provide comprehensive health management to clients of the Nottawaseppi Huron Band of the Potawatomi. The clinician will have a collaborative prescriptive agreement with the Medical Director, and utilize this physician as a resource for practice and clinical inquiries. This position requires a versatile, adaptable and resourceful person willing to take innovative approaches to health care delivery, and to work collaboratively with other care providers and health staff. These services will include health promotion, disease prevention, evaluation, diagnosis and treatment within the values and culture of this community. The community is representative of a diverse medical population with a focus that ranges from family medicine to internal medicine concerns. ESSENTIAL FUNCTIONS The Nottawaseppi Huron Band of the Potawatomi reserves the right to change, amend, add, delete and otherwise assign any and all duties, responsibilities and position titles as it deems necessary to meet the needs of the government. 1. Provides comprehensive assessment of client medical, social and psychological health, including those with acute and chronic disease. 2. Coordinates and evaluates in collaboration with the client and family and their primary care and specialty care providers to obtain optimal health and independence. 3. Conducts comprehensive medical, social and psychological history of clients. 4. Conducts physical exams of clients, either comprehensive or problem focused. 5. Orders, performs, and interprets diagnostic tests for screening and diagnosing, such as xrays, electrocardiograms, and laboratory studies. 6. Develops appropriate formulary. 7. Prescribes medications within regulations of licensure, and if holds a DEA license for schedule II-V drugs will only administer facility appropriate medications. Page 1 of 4 Date 5.20.11 HAM 8. Refers clients to appropriate specialist nurses, physicians, or other health care providers or other NHBP staff. 9. Develops plans of care in collaboration with the client/family and develops mutually accepted strategies for accomplishing health care goals. 10. Maintains a high level of communication and consultation with other health care providers about NHBP clients, especially those who have been newly diagnosed with a disease, have medication changes or been admitted to hospital, home care, hospice/palliative care, rehabilitation, or nursing home. 11. Conducts and coordinates various direct care clinics as needed. 12. Makes client home visits and provides nursing care in the home. 13. Provides information, instruction, and counseling on health maintenance, health promotion, social problems, and medication use. 14. Onsite during dental procedure days to provide medical and or emergency support if needed in collaboration with the dental provider. 15. Evaluates the effectiveness of instruction and counseling and provides additional instruction and counseling as necessary. 16. Teaches groups, families and individual clients about health related topics. 17. Participates in developing pertinent health education materials. 18. Communicates on behalf of clients with insurers, employers, government agencies, and other health care providers, including the NHBP Contract Health Department. 19. Communicates the health care needs of the NHBP in the larger community. 20. Attend patient’s primary care visits to increase collaboration and care. 21. Develop in collaboration with administration and other direct care team members the vision and strategic plan for the NHBP Health Clinic. 22. Develop working relationships with collaborating health organizations, and other health care providers or physician groups assisting with the NHBP Health Clinic. 23. Works collaboratively with other health care professionals and NHBP health department staff to ensure patients receive appropriate care. Intentionally engages and networks with patient’s primary care physicians or specialists to maximize health promotion, disease prevention, evaluation, diagnosis and treatment. 24. Participates in multi-disciplinary team case management meetings. 25. Develops and maintains policies and regulations relative to Indian Health Services (IHS) and the Tribe. 26. Attends billing and coding meetings as necessary, including E&M coding and visit documentation. 27. In collaboration with the health director and medical director, helps to establish and maintain new and existing clinic policy and procedures. 28. Represents the NHBP clinics and staff in a positive manner, within the community. 29. Perform other duties as assigned. MINIMUM REQUIREMENTS An applicant’s education, training and experience must be sufficient to demonstrate that the applicant possesses the ability to successfully perform each of the essential functions. Baccalaureate Degree in Nursing from an accredited school of nursing. 2. Licensed to practice nursing in the State of Michigan with no actions currently pending investigation. 3. Master’s Degree in Nursing, Nurse Practitioner, from an accredited school of nursing in Family or Internal medicine. 4. Certification by the American Nursing Credentialing Center to practice as a Nurse Practitioner. 5. Ability to understand, gain knowledge and appreciate the differences with the Native American culture and customs. 6. Must maintain a valid driver’s license with good driving record; GSA certifiable. 7. Must be able to travel. 8. Must be able to successfully pass a background check: must not have been found guilty of, or pled guilty or no contest to, any felony or misdemeanor offense under federal, state or tribal law involving crimes of violence, sexual assault, molestation, prostitution, or other crimes against persons, or offenses committed against children; must be able to successfully pass a drug screening by means of hair sample. 9. Ability to become CPR/First Aid certified within first 2 months of employment. 10. The ability to work within a collaborative prescriptive agreement with a licensed medical physician and to work collaboratively with patients primary care providers. 11. Computer proficiency, including word processing, spreadsheets, email and internet. 12. The ability to chart and record patient health information and issues in an accepted format and time frame. 13. Demonstrated ability to adherence to ethical codes and confidentiality. 14. The capability to meet or exceed standards for regulations, such as HIPAA and Accreditation Association for Ambulatory Health Care. 15. Proven organizational and time management skills. 16. Maintain an attitude of flexibility, innovation, and a willingness to do other assigned tasks that may occur in a small organization. 19. Possess or will obtain after hire a federal DEA license for schedule III-V controlled substances. 20. Demonstrates knowledge of clinical skill and nursing expertise as an expert Advance Practice Registered Nurse. 21. Act as resource/subject matter expert for the discipline of Nursing. PREFERRED QUALIFICATIONS 1. At least three years of experience as a Nurse Practitioner with experience in community health care and cultural diversity.
