Dear Parent/Guardian:
It has been a privilege to have your student in our school choir this year. As a result of his/her
participation, we are planning a trip to Six Flags Fiesta Texas – San Antonio. We will travel with CCJHS
band. We must have complete cooperation from your student in class and during concerts in order for
him/her to attend this event. The cost of the ticket is $80.00 which includes a park ticket, choir contest,
bus transportation and a supper meal voucher at the park. The total fee must be paid by March 7, 2014
– NO EXCEPTIONS. If for any reason your student is unable to attend after paying, you will be given a
park ticket for your use at a later date.
Signed permission slips must be turned in no later than Monday, April 28, 2014. Your child’s
attendance will be based on behavior in school and participation. Students not passing all subjects (UIL
eligibility) will not perform with the choir. All students must wear their blue choir t-shirts and modest
shorts.Please note that you will be responsible for transporting your child to and from school.

If your parent wishes to chaperone, a CCISD back ground check is required. This can be completed in the CCJHS
front office. The cost for chaperones is $40.00 which includes bus transportation, park ticket and supper meal
voucher at the park. Chaperones will also need to complete the following form and pay in full by March 7, 2014

San Antonio CCJHS CHOIR Permission Slip
__________________________________________ has my permission to participate in the Six Flags
Fiesta Texas-San Antonio choir trip/performance on Saturday, May 10, 2014 and has my permission to
accompany the school sponsors on this out-of town choir trip. My son/daughter has assured me that
he/she will exhibit conduct that will be a credit to Copperas Cove Junior High school and the
organization represented. I understand that all school rules, policies and guidelines apply to the
students and sponsors who are involved. You may be assured that my child understands the rules and is
being permitted to accompany the choir on this bus trip with my full consent.
Students will be chaperoned and normal precautions will be taken in the interest of student safety and
well being. I agree that the faculty advisor, chaperones and the school will not be held responsible for
any accident or misfortune which may occur in connection with this trip.
I understand that I must transport my child to CCJHS at 6:00 a.m. and pick them up at 1:00 a.m. on May
10, 2014. I also understand that my child’s behavior, class work and attendance must be UIL satisfactory
in order to participate.
