They are likely to complete the maintenance upto Kalta from Rajamunda side by the end of May. Traffic diversion plans have not been made at the critical locations. 2. Neither advance warning signs nor proper barricading is arranged. Inspection Report May - 2014 0.75mts interval. 3. Retro-reflective tape should be pasted on sand bags for night visibility. As per contract agreement, maintenance of existing road is the responsibility of contractor. Repair of all pot holes and cracks should be undertaken on regular basis and the riding quality need to be improved. Water should be sprinkled regularly to control the dust pollution and to enhance the riding quality of existing surface during execution of earthwork and GSB etc. Since the movement of heavy trucks is more particularly at night time it is advisable to provide intersection signs for entire project road as per IRC with retroreflective sheets for night time visibility. Traffic diversion plan should be prepared as per IRC:55-2001 “Guidelines on safety in road construction zones” and submitted to the safety auditors for review/finalisation. KM 251+400. 6 Improper barricading and guard fencing at places of KM 169+300, construction activity KM 170+100 & alongwith unprotected edge drops. KM 171+200 The area should be properly barricaded as per IRC SP-55 type III barricades. The work zone signs should be provided as per IRC SP55. 7 KM 245+600 To prevent traffic from entering KM 166+900, NHAI No protection is provided Unihorn India Pvt. Ltd. Inspection Report May- 2014 the area, unprotected culverts should be barricaded as per IRC:SP55. Incomplete construction work should be completed on urgent basis for such locations. Diversion sign should confirm to IRC:55 clause 3.2.7 KM 239+000, Substandard warning signs and diversion signs KM 247+900 & which does not confirm to IRC. KM 250+000 KM 238+000, 9 10 KM 190+000 No flagman observed at construction site for loading and unloading. 1. Inadequate barricading and fencing. KM 245+980 & 2. No advance warning signs on approaches KM 246+000 of railway crossings have been placed. KM 184+800, 11 All warning signs and diversion sign should confirm to IRC:55 clause 3.2,3.2.7 It is desirable to deloy a flagman for loading and unloading the construction materials as per IRC:55 clause 3.5. 1. At locations of wedged up trucks, appropriate signs should be provided. 2. Proper barricading/fencing should be provided to avoid fall of any vehicle at construction zones. 3. Advance warning signs should be provided on all approaches of railway crossing. KM 177+500, KM 175+700, KM 186+500, 12 KM 186+700, Advance curve signs have not been provided. KM 165+700, 13 Physical objects lying on the ground on road side should be removed immediately. KM 244+300. 14 Trees which are comming within the formation KM 202+700 & width does not have retroreflective paste/ KM 207+000 stickers. This is hazardous to road users. Retro-reflective tape should be pasted on the trees comming within the formation width. 15 Absence of retroreflective stickers on sand bags with poor night time visibility. Good quality retroreflective stickers should be pasted on all sand bags. 16 Poor night time visibility & substandard use of signs which do not confirm to IRC. Retro-reflective standard signs as specified in IRC:67-2012 should be installed on the project corridor. KM 164+600, NHAI 17 Workmen working without proper safety gears. 18 Mechanical equipments are unguarded with protective coverings and barricading. 19 Worn out power cords are there at workzones and absence of proper electrical safety practices for handling equipments. 20 Fire extinguishers are absent at petrol pumps, workzones and inadequate storage facilities for inflammable materials. 21 Inadequate workzone signs traffic control measures at diversionzones. IE for 4/2 Laning of Rimuli –Roxy- Rajamunda section of NH-215 from km 163+00 To km 259+453 in the state of Orissa On DBFOT Basis under NHDP-Phase- III. 13 Site inspection Report NHAI Inspection Report May- 2014 Section- 1 M/s McNally Bharat Engineering Company Ltd. has withdrawn and moved out all the available resources in section-I. Presently no work is found in progress in section-I.M/S OSEPL and NHAI may take necessary action so that working season can be availed for execution of maximum works. Section- 2 1. EPC for Section-2 is now attending the maintenance of the existing road for the stretch km 201+900 to km 239+000. They are likely to complete the maintenance of the existing road from Kalta to Lohunipada by end of May 2014. 2. Work on the Koida bypass has been again taken up and excavation for P1,P2 piers and A2 abutment foundations of a major bridge on Koida bypass is in progress. Also setting up new camp on Koida bypass is in progress. 3. Maximum resources are found concentrated in the stretch km 239+000 to km 259+453 to achieve the laid down targets and now another camp near Kairo Nallah at village Kusumudi is being established so that construction of Kairo bridge and Koida Bypass works can be taken up. 4. Kerbs casted for the median have now been provided with saw cuts at every 5 mtrs. 5. Filling material in the median has now improved. 6. EPC is having only one Bitumen pressure distributor. They should have two bitumen pressure distributors, i.e. one for the slow setting emulsion for the prime coat and the other for the rapid setting or medium setting emulsions for the tack coat. EPC should do the needful now. 7. EPC should make one asphalt cutter available for making proper jointing for starting the daily work. 8. Quite longer lengths of WMM have been found completed on ground. The same should be covered with the next layer of DBM otherwise finished WMM surface may develop deformations.EPC should take immediate action to cover the finished WMM surface with the DBM layer. 9. All WMM work must be executed through a WMM paver with sensor device. Laying WMM with motor grader will not be accepted. 10. EPC has now started procuring VG-30 from M/s HPCL. 11. All the joints in the DBM surface have not been prepared properly .There are big undulations created at the joints. Necessary corrections may be carried out now. 12. The desired quantity of water was not being used for the earthwork. This was shown to the rep of the EPC and advised to improve the quality of earthwork. 13. Filter layer was now provided while filling behind the culvert walls. 14. Both avenues are full with surplus soil. The rep of the EPC was advised to either remove this surplus soil or level the same within the ROW area which will permit easy drainage of the area. 15. Filling behind Minor Bridge is now taken up after giving the layout of the construction area. Toe walls for the quadrant portions should be completed and filling in the quadrant portions should also be taken up simultaneously. 16. Quality of concrete is now improving. 17. EPC has now made available one concrete pump for placing the concrete at different levels. 18. Rep of the EPC was advised for taking up the development of Major Junction with NH-143 also since Lahunipada bypass is likely to be completed shortly. 19. Lab establishment is completed and procured all apparatus. 20. Both crushers have been established at Base camp at km 252+000 and production of aggregates is in progress. Unihorn India Pvt. Ltd. Borrow areas for Earthwork and sub grade materials sampling have been collected and testing completed. 22. Aggregates production is in progress. 23. Concrete mix designs for M-15, M-20, M-25, M-30,M-35 & M-40 have been completed. 24. Camp construction is already completed. Sufficient manpower deployed at site. 25. Tree cutting has been stopped. 26. Shifting of electrical poles is in progress but progress is slow. 27. Removal of hand pumps not yet taken up. 28. Very little work for payment of arbitration amount is now balance and EPC is already taking the required action. 29. Over all progress of works is very slow. 30. The process for obtaining approval of GAD for all the three ROBs has not yet been completed. 31. Construction work of ROB at Km 245+950 has been stopped. 32. The concessionaire is coordinating the forest clearance from the MOEF. The critical activity in this process is cutting a huge quantity of about 55000 no. of trees. 33. The stretch of the existing road from km 206+000 to km 219+000 is still in very poor condition. Maintenance of this portion should also be attended immediately. 34.
