Please note there is a fee for this service and it can take up to 8 weeks for completion.
Directions for Current Virginia Tech HNFE Dietetics option students or HNFE alumni:
Current HNFE Dietetic seniors or HNFE alumni requesting either paper copies or electronic DICAS versions of a
“Declaration of Intent to Complete” form or a “Verification Statement” must:
1. Complete page 2, listing any university or colleges attended;
2. Complete page 3;
3. Fill in any course substitutes on page 4. Leave the page blank if you do not have any course substitutes;
4. Staple all pages together and submit to Sherry Saville (address below).
5. Fill in any courses left to take at VT on page 5;
It is not necessary for you to obtain a copy of your Virginia Tech transcript. We will let you know if we
need official copies of transcripts from any other university or college.
Fee for Virginia Tech HNFE Students and Alumni: $25.00
Your student account will be charged $25.00 if you have completed courses in the Department of Human Nutrition,
Foods and Exercise (HNFE) at Virginia Tech. This includes individuals who have officially transferred courses to
complete a degree or are currently enrolled in a degree program in the HNFE Department. This also applies to
incoming graduate students wanting to finish DPD requirements. Once your student account is settled, you will be
notified via email to pick-up any Original (paper) copies of either the “Declaration of Intent to Complete” or “Verification
Statement” and/or your electronic DICAS documents will be completed by the DPD Director.
Note:The Department of Human Nutrition, Foods and Exercise no longer honors requests for Verification Statements for Plan IV, effective Jan. 1990.
Directions for Non-Virginia Tech HNFE students or Non- Virginia Tech HNFE alumni:
Prospective students who would like their transcripts reviewed and a tentative plan of study developed to complete a DPD
Verification Statement at Virginia Tech must:
1. Complete page 2, listing any university or colleges attended;
2. Leave pages 3, 4 and 5 blank;
3. Send official transcripts from the Registrar’s office of any university or college you would like to have courses
reviewed for progress towards your Verification Statement (address below).

You will receive an e-bill for this charge so please include an active email address. Please do not send a check or
money as this is against university policies.
**Important: The DPD transcript evaluation process is completely separate from the admission process to Virginia Tech. You must apply to
