General Template Information and Instructions

Instructions in this template are set out in grey font.
•
The terms and conditions of the OECM Master Agreements govern this request for services.
•
The usage of the request for services is for evaluation and assessment of the supplier offering, customers
are not obligated to any subsequent purchases or to award the request for services.
•
The request for services does not create a Contract A/ Contract B situation.
•
Customer’s should review and assess if the cloud technology services; IT professional services; IT
technical support services; networking switches, routers, firewalls products and related services and/or
software licensing in our master agreements meet your needs. If they do, customer can purchase directly
from one or more of the suppliers using the rates set out in the master agreements, alternatively, customer
could issue a request for services to obtain services/products and/or pricing based on your specific
requirements.

Consult your internal policy to see if the supplier is required to sign a Confidentiality Agreement before
receiving your specific information.
•
Fill out the fields under Customer Details and Customer Requirements.
•
Once you completed the assessment of the supplier offering and decide to purchase, please ensure a
Client Supplier Agreement is signed between the selected supplier and your organization.
Background Information:
Late 2014, OECM reached out to its customers to complete the sourcing project survey 2015. With this survey
OECM was asking customers to provide the list of projects they wanted OECM to undertake in 2015. Customers
expressed interest in IT Disaster Recovery Services.
Following its regular procurement process, OECM consulted with various suppliers and formed an Advisory
group. During the supplier consultations, OECM realized that the IT disaster recovery services could be acquired
using some of OECM’s existing IT agreements.
In the fall of 2015, it was agreed to not pursue an RFP for the provision of IT disaster recovery services but to
create a package of services from existing agreements.
Customers can utilize the following agreements to acquire IT disaster recovery services:

IT Professional Services 2014-MA-205 under the following categories:

Audit and Compliance;

Electronic Record Management Services;

IT Infrastructure;

Managed Security; and

Network Services.
IT Technical Support Services 2014-MA-213;
Networking Switches, Routers, Firewalls and Related Services 2014-MA-206; and
Software Licensing 2014-MA-228.
Instructions:
Using this request for services template, the client should clearly outline the required IT Disaster Recovery
Services to enable the supplier to provide a quotation specific to the requirements. A client should complete
sections 1 and 2. The invited suppliers should provide their responses using sections 3 and 4.
The request for services should be sent to the suppliers’ client representative, contact information can be found in
appendix A. Clients do not have to sign a Client Supplier Agreement to issue and submit a request for services.

4.4 - Execution of the Request for services
Instructions:
In table 4.4 the supplier shall complete and sign its request for services response before submitting it to clients.
If this request for services response is selected by the client, the supplier agrees to provide the services
described in the table 4.3 at the price indicated in table 5.3. The supplier shall make its total price excluding
HST (section 5.3) valid for ninety (90) days.

The supplier should submit its quote to the client via email at the address indicated in section 1 and 2,
before the request for services expected response date stated in section 1.
If the supplier is selected to provide the services, a Client Supplier Agreement (CSA) must be executed
prior to the provision of any services. Therefore, if the client and supplier have not already done this, the
CSA must be completed and signed if the client awards the services from this request for services.
