Partners in Support JOB DESCRIPTION - TEAM LEADER This job description is a statement of the core duties of a Team Leader. Each individual supported will also have a person specification that will outline the key characteristics, skills and qualities of the staff that will be recruited to provide their support. The role of the Team Leader is pivotal to leading and ensuring the values of Partners in Support are maintained and developed. Role and Responsibility The core responsibility of the Team Leader is to lead, coordinate and facilitate the support provided to an individual. This will include the leadership and management of the dedicated staff team/s. They will be expected to participate in a management on-call rota for Partners in Support. The role requires the ability to use initiative and to work independently and creatively. The Team Leader must ensure that the individual is enabled to direct their support and live the life that they wish, working closely with their family at all times. The role also involves ensuring that the person continues to develop / progress and establish and maintain positive relationships with members of their community with an aim to extending natural support and reducing dependence on paid support. The Team Leader will be responsible for ensuring compliance with all legal and organisational requirements and for ensuring excellence in areas which are measured internally and externally. Main Responsibilities To ensure the individual receives the highest quality of support in line with the values of Partners in Support. The Team Leader will 1. ensure that the Team supports the individual in a consistent and person centred manner in accordance with their wishes and needs 2. develop a good understanding of the person and support the continuous development of accessible communication that makes sense to the individual and arrange staff training and induction where necessary 3. lead the team in ensuring that the individual is empowered and supported in a creative and thoughtful manner which suits them best. 4. lead the team to find potential in every moment for the person to make decisions and take control on a day to day basis in all aspects of their lives in. 5. work in partnership and ensure external communication is effective with the individual, their family, friends and all relevant professionals 6. work using initiative, finding proactive solutions and ideas as well as ensuring positive and consistent reactive strategies are in place to support individuals 7. adhere to and implement Partners in Support policies and procedures as well as external polices which apply to the individual and the team, and to ensure that all compliance standards are met 8. take lead responsibility for ensuring that the individual’s physical, emotional and mental well-being is considered and prioritised, taking appropriate action when necessary and to liaise with professionals as required 9. ensure any concerns regarding the individual are reported to the Service Manager, Assistant Director and/or Director 10. support, create and develop a positive vision for the person using person centred thinking tools to facilitate this where relevant 11. develop positive, person centred action plans with the person to support and monitor their personal development Key Duties 12. To lead and support the team to monitor the individual’s mental, emotional and physical health and take appropriate action to safeguard and maintain their health 13. To lead and support the individual and team to develop and maintain relationships with neighbours, their friends and family 14. To develop a clear and consistent internal communication system e.g. diaries, communication books 15. To develop effective external communication systems with relatives, professionals and other external partners 16. To problem solve with the team and develop and implement proactive person centred risk management approaches 17. To ensure the individual has support organised to meet their needs, including the arrangement of flexible and creative rotas 18. To coordinate the recruitment of staff and include the individual and their family in this process 19. To provide regular supervision for staff and facilitate regularly team meetings 20. To provide induction for staff and training as required 21. To manage the performance of individuals and the team to ensure best possible results for the individual 22. To ensure that the individual is supported to take responsibility for their individual finances and that transactions are recorded 23. To assist the individual to maximise their benefits and gain assistance from Welfare Rights agencies where necessary 24. To be responsible for the management of the team budget and ensure hours are available within budget and managed effectively The responsibilities of the post holder are reviewed regularly and subject to change where necessary. Last updated October 2015 Partners in Support PERSON SPECIFICATION - TEAM LEADER Location: Hemel Hempstead Salary: £13,447 per annum Reporting to: Service Manager Sleep-in Allowance: £32.00 per night EXPERIENCE 1 Two years experience of providing person centred support to people with learning disabilities and/or Autism Essential 2 Experience of working positively with families and professionals Essential 3 Experience of working with people who may challenge Essential SKILLS 4 Evidence of networking skills and ability to build links within Watford Essential 5 Ability to manage difficult situations calmly and effectively Essential 6 Ability to work independently and use own initiative Essential 7 Ability to motivate and lead a team of dedicated staff Essential KNOWLEDGE 8 Knowledge and commitment to Person Centred Approaches and the values of Partners in Support Essential 9 Possession of, or commitment to obtain, an appropriate Diploma in Health and Social Care Essential INTERESTS AND CHARACTERISTICS 10 Motivated and confident, being able to support activities that are relevant to the person being supported Essential 11 Calm, patient and positive in their interaction at all times Essential CIRCUMSTANCES 12 Able to work flexibly, including weekends, evenings and sleep-ins Essential 13 Car Driver Essential
