On January 12, 2014, at about 6:30 PM, a telephone call was received by Talavera PS from SPO1
Ernesto J Nagano of COMPAC Bacal District of said station informing that a male cadaver was found dead
at Barangay Bacal II, Talavera, Nueva Ecija. Initial investigation disclosed that at about 5:00 PM, same date,
the victim identified as Oscar Bonducan @ “MASTER”, legal age, power plant worker and a resident of
Fairview, Quezon City was found lifeless inside their bunkhouse located at said barangay. Further
investigation disclosed that the victim was discovered by his co-worker identified as Jayson Blanco y
Taguba, 30 years old, single and resident of Barangay Linao, Aparri, Cagayan when he tried to awaken the
victim who was then sleeping inside their bunkhouse but the latter did not respond. Pictures of the scene
showing no struggle nor indications of foul play were taken. The cadaver of the victim was brought to Senior
Citizens Funeral Parlor for autopsy examination. Investigator on Case: SPO1 Ernesto J Nagano with CP#

On January 12, 2015 at about 9:PM, elements of Llanera PS led by PSINSP JAIME PALTAO
FERRER, COP, while conducting anti-criminality checkpoint along Rizal-Pinagpanaan provincial road, Brgy.
Mabini, Llanera, Nueva Ecija arrested Ronnie Moreno y Uraga, 32 years old, married resident of Brgy.
Villarica, Pantabangan Nueva Ecija for Violation of RA 8294 as amended by RA 10591 (Unlawful Acquisition
or Possession of Firearms and Ammunition). Investigation disclosed that the arrested suspect was onboard
a Toyota Corolla color blue gray, bearing plate number THT 552 registered in the name of Augusto M. San
Diego and traversing Rizal-Pinagpanaan Provincial road heading north direction (to Rizal), Brgy. Mabini,
Llanera, Nueva Ecija. The troops conducting checkpoint at the said barangay flagged down the subject
person for plain view inspection. The troops immediately noticed through plain view inspection a firearm
placed on the hand brake of his driven vehicle. Subject person failed to present Licensed and Permit to
Carry Firearms Outside Residence (PTCFOR). Recovered from the suspect is one (1) Viper 2 9mm pistol
with serial number 12275, one (1) magazine with seven (7) live ammunition and holster. The arrested
suspect together with the confiscated firearm was brought to Llanera PS for proper disposition. A violation
of RA 10591 (Unlawful Acquisition or Possession of Firearms and Ammunition) is being prepared by this

On January 12, 2015 at about 6:00 PM, a vehicular accident transpired along Cabanatuan-Papaya
Road, in front of Catalon Residence in Pitong Gatang, Brgy. Sta Arcadia, Cabanatuan City involving V1
Honda Beat motorcycle colored black/grey, bearing plate number 8172 YO, being driven by JESSIE
SALAZAR y CRUZ, 42 years old, married, Laboratory Technician, resident of Purok 7, Brgy. Sta Arcadia,
Cabanatuan City, no CR/OR of vehicle and driver’s license presented and V2 Motor Star motorcycle colored
red, without license plate displayed, being driven by JEFFREY VICENCIO y FORTUNA, 22 years old,
single, carpenter, resident of same place, no CR/OR of vehicle and driver’s license presented. Investigation
disclosed that V1 was then travelling on said road bound to south direction (to Tagpos, Sta Rosa) while V2
was running on opposite direction ( to north ) and upon reaching the place of incident V1 accidentally
bumped V2 while the former was on the process of overtaking to the left . At this juncture, both vehicles
were crashed on the road. As a result thereof, both drivers sustained injuries on the different parts of their
bodies and were brought to Dr. PJGMRMC for medical treatment while their driven vehicles incurred

On January 12, 2015 at about 2:45 PM, a vehicular accident transpired at the intersection of Felipe
Vergara High Way and Cabanatuan City-Carmen Road, Brgy. Cinco-Cinco, Cabanatuan City involving V1
Hyundai Accent,colored red, bearing plate number RLY 565, owned and driven by KATHERINE LAPUZ y
DE LEON, 32 years old, single, Municipal Director of DILG San Leonardo, Nueva Ecija, resident of Brgy.
Malaya, Sto Domingo, Nueva Ecija, holder of Non-Professional Driver’s License with no. C05-05-002455
and V2 Kawasaki Bajaj motorized tricycle, without license plate displayed, bearing body no. QT 313, driven
by MARK ANTHONY AQUINO y TARANTE, 31 years old, married, tricycle driver, resident of no. 214 Purok
4, Brgy Imelda, Cabanatuan City, holder of LTO student permit. Investigation disclosed that V 1 was then
cruising the said intersection bound to east direction ( to Robinsons Mall ) while V2 was on the process of
turning left came from north direction and bound to east direction when both vehicles collided. As a result
thereof, the left side front bumper of V1 incurred damages of still undetermined amount. No one was hurt
during the incident. After confrontation to Cabanatuan City PS by both drivers involved, they were agreed on

On January 13, 2015 at about 8:30 AM, a cell phone call was received from PO3 Aquilino Aguirre,
member of Munoz City PS informing that a robbery incident transpired in Muñoz Traveler’s Multi-Purpose
Cooperative in the compound of Gov. Eduardo L. Joson Memorial Lodge no. 326, Brgy Maligaya, Science
City of Muñoz, Nueva Ecija, between 10:00 PM of January 12, 2015 to 4:00 AM of January 13, 2015. Upon
receipt of information, patrol team with duty investigator dispatched and proceeded at said place of incident
to conduct investigation. Investigation disclosed that on January 13, 2015 at about 5:00 AM, Mario Dela
Cruz y Santiago, 55 years old, Board of Director of Muñoz Traveler’s Multi-Purpose Cooperative, married
and a resident of Purok 1, Maligaya, Science City of Munoz, was about to visit his rice field when he noticed
that the said Traveler’s Multi-Purpose Cooperative Bldg was lighted. Suspecting that unusual incident
transpired, since it was not usually left lighted, it prompted him to go back and checked said establishment.
He was surprised when he discovered that the padlock of the steel matting door was forcibly broken. Once
inside, he conducted inspection and inventory of all their valuable items therein. As a result of the conducted
inspection and inventory he learned that the following items to wit: desktop computer, CPU, monitor,
keyboard (amounting to Php10,000.00; 40 inches brand new colored TV (amounting to Php 28,000.00); TV
of videoke, videoke player, amplifier and three speakers of videoke (amounting to Php25,000.00); home
theater speaker and DVD player (amounting to Php 5,000), were forcibly taken and carted away by
unidentified suspect/s. Follow up investigation is being undertaken to ascertain the identity of the suspect/s.

SUBJECT : Progress Report on Frustrated Murder

On January 9, 2015 at about 7:30 PM, a shooting incident (Frustrated murder) transpired at Brgy
Tikiw, San Antonio, NE wherein the victims were identified as one MICHAEL VILLAS y SALONGA, 35
years old, married, Barangay Councilor and MANUEL AZUCENA y MACATUNAO, 33 years old, male,
married and both resident of Brgy Tikiw, San Antonio, NE while the suspects were one Alias HONEY
SEBASTIAN and Alias CHARLEN ALVAREZ both male and residents of Brgy San Mariano, San Antonio,
Nueva Ecija. Furthermore, in-depth investigation disclosed that the companion of the suspect, HONEY
SEBASTIAN, during the incident was his brother, identified as ALFIE SEBASTIAN, who participated as the
driver of the single motorcycle used during the crime. On January 12, 2015, a criminal complaint for
DOUBLE FRUSTRATED MURDER (Shooting Incident) was charged against the suspects and filed before
the Office of the Provincial Prosecutor, Cabanatuan City, Nueva Ecija and docketed under NPS No. III-09INV-15A-00062. For information.

On January 12, 2015 at about 7:45 PM, LEODY ESPIRITU y TORRES, 25 years old, single,
resident of Purok Camia, Brgy. DS Garcia, Cabanatuan City was arrested and brought to Cabanatuan City
PS by the security guard of 7/11 convenience store, after the former surreptitiously stole three (3) pieces of
AXE deodorants and two (2) pieces of GATSBY STYLING WAX amounting to Php 519 from the selling area
of said establishment. A criminal complaint for the above-cited case against the suspect was already

SUBJECT : Spot Report on Carnapping Incident

Reference: Memorandum from COP, Cabanatuan City PS dated January 12, 2015.
On January 12, 2015 at about 11:30 PM, above-cited incident transpired in Purok 2, Brgy. Bakero,
Cabanatuan City wherein the Kawasaki Bajaj motorized tricycle, colored black, with Plate No. QV-6966,
being driven by ROBERTO RANGCAPAN y DELOS SANTOS, 34 years old, married, tricycle driver, and
resident of Purok 7, Brgy. Bagong Sikat, Cabanatuan City, was forcibly stolen by his unidentified male
passenger which he fetched in Magsaysay Market Complex, Brgy Magsaysay Norte, Cabanatuan City.
During the process, the suspect shot the victim, hitting the latter on his abdomen and thigh. The victim was
rushed to NE Good Samaritan Health System Inc., for medical treatment while the suspect immediately fled

LUCIANO MATIAS y ORTIZ, 44 years old, single; ALVIN ESPIRITU y FLORES, 19 years old, single;
RAFAEL ALIPIO y ENTENDENCIA, 28 years old, single; RAYMARK LAUSA y LAO, 23 years old, single;
ROMAN DE JESUS y JERILLA, 39 years old, married; ROMEO FELICIANO y DELA CRUZ, 37 years old;
RICARDO PINTO y IÑIGO, 24 years old, all residents of Brgy. DS Garcia, Cabanatuan City and ENRICO
EUSEBIO y LIWAG, 37 years old, single, resident of Purok Amihan, Brgy. Barrera, Cabanatuan City were
arrested and brought to Cabanatuan City PS by PO1 APOLLO V JULIANO of Provincial Intelligence Branch,
NEPPO and personnel of COMPAC 512, this Police Station, who caught the above-mentioned persons
while in the act of gambling/ playing CARA y CRUZ. Recovered during the arrest was the bet money
amounting to PhP 1,585 cash. A criminal complaint for the above-cited case against the suspects was
already referred to Cabanatuan City duty Inquest Prosecutor Francisco U Macaraig docketed under NPS

Mr. RODOLFO DE LEON y PACLAON, in Milflores St., Brgy. Mayapyap Norte, Cabanatuan City. Subject
vintage bomb was subsequently turned-over to the custody of 15th EODT, based in Fort Magsaysay,

On January 13, 2015 at about 3:30PM, theft incident transpired at Gina Angeles Dallo Palay and
Rice Trading, located at Mommy Lola building, D. Delos Santos St., Brgy. Pob. West, Science City of
Muñoz. Victim was identified as Gina Angeles Dallo, 46y/o, married, proprietor of Gina Angeles Dallo Palay
and Rice Trading, and a resident of road 1, Teacher’s Village, Brgy. Pob. South, Science City of Muñoz;
while the suspect was identified as one Alvin Tuazon Soriano, 37y/o, single, tricycle driver, and a resident of
Purok Curva, Brgy. Bantug, Science City of Muñoz.
Investigation disclosed that a customer was then about to buy six (6) cavans of twenty five (25) kilograms of
rice from the victim. Thereafter, the customer called a tricycle to load and transport the said rice. When the
helper loaded one (1) cavan of rice to the said tricycle immediately its driver sped up and drove towards east
direction. In that juncture, the victim’s helper droved a tricycle and chased the suspect however he failed to
apprehend the latter. Having remembered the body number of the said tricycle identified as number 1252
the victim’s helper conducted searched at all the tricycle terminal at the public market. Hence, said helper
was able to locate the said suspect and seek the assistance of the duty Traffic Enforcers thereat to arrest
the suspect. Suspect was brought at Munoz City PS for proper disposition. A case for the crime of theft is

SUBJECT : Spot Report on Traffic Accident

Reference: Memorandum from COP, Guimba PS dated January 13, 2015.
On January 13, 2015 at about 7:35 PM, a concerned citizen Guimba PS regarding pedestrian
accident transpired in Barangay Cabaruan, Guimba, Nueva Ecija. Immediately thereafter, PO2 Jaycee B
Toribio and PO2 Rowell F De Guzman proceeded to the said place to verify the report and to conduct
investigation. Upon arrival thereat, the victim identified as Ronnie Fabros y Balintag, 48 years old, married
and a resident of Brgy. Cabaruan, Guimba, Nueva Ecija, the driver of involved motorcycle described as Rusi
color blue/gray with plate # CA 22172 identified as Robin Del Rosario y Reyes, 23 years old, single and a
resident of Brgy. Baloc, Sto. Domingo, Nueva Ecija were already brought in Guimba General Hospital.
Investigation disclosed that at about 7:20 PM this date, while Robin Del Rosario y Reyes driven his above
described motorcycle was on his way home in Baloc Sto. Domingo, Nueva Ecija and upon reaching within
the vicinity of Brgy. Cabaruan, Guimba, Nueva Ecija, the victim suddenly crossed the road which tried to
avoid by the former but unluckily hit/bumped the victim and therafter, the former slide on the pavement.The
victim sustained serious injuries and still unconscious while the driver sustained minor injuries.IOC PO2

MD2DDDUZZPWM19918, with sidecar volvo half Stainless/half galvanize, registered to Mr. Eustaquio
Untalan y San Jose, legal age and a resident of Brgy. San Pedro, Gen. Tinio, Nueva Ecija and was owned
by Arthur Untalan y San Jose, 36 years old, married, farmer, a resident of Sitio Balimbing, Brgy. Sinasajan,
Peñaranda, Nueva Ecija. Subject motorcycle was parked unattended inside the owner’s residence and was
last seen OOA 11:00 PM, January 12, 2015 and was discovered missing on January 13, 2015 at about 3:00
AM, it was believed to be taken/carted away by unidentified suspect/s. If found/apprehend, kindly inform this
