Behavior Models and Behavior Change Assignment
PART A:
Health Behavioral Change Models
As studied in class, there are many health behavioral change models used to help people
make decisions that can affect both short-term and long-term health and wellness. Four
specific models, as described in your notes and textbook, include:

Complete the following information (based on targeting a health behavioral change) of:
ELIMINATING BINGE DRINKING.

2. A detailed list, steps, or summary of the strategies that can be used in the model for
successful change of the behavior.
3. Provide a summary of which model you feel is the most successful model and which
model you feel is the least successful and explain why.
PART B:
Health Behavior Change Plan:
Each student will assess their health behavior status, pick a health behavior to change, (a good
reference to identify a behavior could be based on the “Assessment” taken at the start of the
semester); complete and sign the Behavior Change Contract (page A-10 in the front of course
text or (hand out)and write a detailed report on their success or challenges. This is an entirely
personal report and requires no references.
Your written Health Behavior Change Plan and progress report should be 4 pages in length. If
you supplement your report with outside resources, please remember to acknowledge them in
your report.
Include in your report:
1. Summarize the behavior to be changed. What changes can you make now? In the near
future?
2. Select one pattern of behavior you’d like to change.
3. Fill out Behavior Change Contract (page A-10 of text or handout).
4. For all of (month) , chart your progress in a journal. At the end of (Month), describe how
successful you were in following your plan. If you weren’t as successful as you’d like, what
barriers might have influenced this? Be specific.
5. Revise your plan as you feel is needed for future. What are your short-term goals? Are they
realistic and possible? Are the rewards you’ve set-up satisfying and motivational for you?

Include a cover sheet, indicating your name, HED-130, title of paper and date due.
Include the “Behavior Change Contract” page
Please, no plastic binders or other covers.
All reports should be stapled prior to being handed-in to instructor.
Be careful about using correct grammar. I encourage students to use “spell check”.

Devise and state a Strategy or Plan of Action and a Method for Tracking (Chart, Graph,
