arrive. Schools will have them complete a “30 day waiver” form. Parent/guardian signs a statement that they agree that when the records from
the other school arrive, if the information in the record is incomplete, they will get the needed immunnizations for their child.
Note: *30 day waivers are not applicable for first time enrollees (ie…kindergarten or pre-kindergarten) or Grade 7.
Homeless students are the only exception to this rule. Florida Statute gives homeless students a 30 day grace period to comply.

September 1, 2005
Dear Parents,
During the summer, notices were sent out requesting student information that is missing from
your child/children’s student file.
We are once again asking that you take care of this important information. The Pasco County
School District and the Pasco County Health Department require all students to have the proper
immunizations, Birth Certificates, and Social Security numbers in their school’s student files.
Please help us with keeping what Pasco County requires by bring to the office what we have
asked for.
Students who do not have this information in their files by October 1, 2005, will not be allowed to
attend classes until we receive all of the forms required.
Sincerely,
Administration
