Health Record Due Date: Bring to Spring into Maroons Orientation

or by August 1 st for fall semester or January 7 th for spring semester o

Tuberculosis Risk Assessment Form

Please note: If indicated PPD skin test or QuantiFeron Tb Gold (QFT-G) blood test

must be within 1 year prior to arrival at Roanoke College. o

Immunizations: Check with your doctor about the following:

Required:

 MMR (measles, mumps, rubella) 2 doses. On or after 1 st birthday and at least

28 days apart

 Tetanus booster (Tdap preferred) must be within past 10 years

 Polio date series (4 doses) unless 3 rd dose was after 4 th birthday

 Hepatitis B: (3 doses or signed waiver meets requirements)

 Meningococcal Quadrivalent (A,C,Y,W-135) vaccine given on or after 16 th birthday or signed waiver meets requirement.

 Varicella (chickenpox) A history of chickenpox disease, 2 doses of vaccine or positive varicella titer meets requirements.

Recommended:

Quadrivalent Human Papillomavirus a 3 dose vaccine series

 Hepatitis A: 2 doses

 Influenza: Flu shot clinics will be announced after fall break o

Health Insurance:

Domestic students review medical insurance and ensure your policy provides adequate coverage

while living in Salem, VA for services such as emergency care, lab tests and x-rays. Carry your

health insurance card and prescription drug card with you. o

Medical records and prescriptions:

Have a record of your prescriptions, including dosages and diagnosis/reason for

treatment. If you have chronic/serious medical problems, have a medical summary sent

to Health Services by your treating physician. **If you are currently prescribed

medication for ADD/ADHD, and will be needing prescription refills while at school,

a letter from your physician with documentation of diagnosis and medication dosage is

required and must be dated within 12 months. o

Allergy shots:

Health Services gives allergy shots at regularly scheduled times by appointment. A physician’s

detailed order is required. If initiating allergy shot therapy, we require the ordering physician give

the first injection. Students must wait 30 minutes after injections. o

First aid supplies:

Bring digital thermometer, over the counter cold and pain relief medications, band-aids, topical

antibacterial cream and a chemical cold pack.
