If you are interested in participating in Basketball or Bowling in the winter, please complete this
registration and send with payment to the school office: Attention CYO.
The participation fee is $135. Checks should be made out to OLQM.
Please note the registration deadline; it is critical that you register on time to ensure that we
register the correct number of teams with the CYO office. All registrations submitted after
September 13th requires a late fee of an additional $15 and you may be placed on a waitlist if teams are full.

* Physical forms must be dated May 1, 2013 or later to be eligible for the entire school year and must be turned
in before the first game. Physical and liability forms are available in the school office or on the school website.
Team placement evaluations and practice will begin November 4th.
