[NOTE: New 1st & 2nd grade teams formed in the summer must inform the SBYB registrar of their intent to play as early as possible.

GAME FORFEITURE
In order to maintain the integrity of the league and to honor the schedules of all of the families involved, if a team forfeits a game due to not enough
players from the a team being present at the scheduled game, the following rules will be applied.
WINS / LOSSES
 Any team forfeiting a game will automatically earn a LOSS in the standings. The non-forfeiting team will earn a WIN in the standings. In grades
1, 2 and 9, there are no standings; therefore, no WINS or LOSSES will be recorded
PENALTY
 1st Forfeiture in the season by a team:
A penalty of $300 will be assessed to the forfeiting team UNLESS the forfeiting team can fulfill two (2) conditions at which a penalty of $100 will
be assessed. 1) The forfeiting team coach or parent rep must call via phone the respective division coordinator by the Thursday at 8:00pm
before the intended forfeited game. 2) The forfeiting team must be able to field a team of a minimum of five (5) players* to "scrimmage" against
the other scheduled team. The five (5) players as referenced must be of same grade level in the division AND have an SBY Player Release
form on file for the current season.
 2nd Forfeiture and any subsequent forfeiture in the season by the same team
A penalty of $300 will be assessed to the forfeiting team. The coach and/or parent rep of the forfeiting team must contact the division
coordinator by the Thursday at 8:00pm before the intended game. The forfeiting team may still field a team of a minimum of five (5) players* to
"scrimmage" against the other scheduled team as long as the said players are of the same grade level in the division AND have and SBY
Player Release form on file for the current seaso

LIABILITY WAIVER, RELEASE, INDEMNITY
AND ASSUMPTION OF THE RISK AGREEMENT
READ BEFORE SIGNING
In consideration of the player’s participation in the basketball events and activities sponsored by South Bay Youth Basketball League, I/we, for
myself, my spouse and the player and our respective heirs, successors, assigns, agents, and administrators do HEREBY RELEASE, WAIVE,
DISCHARGE, INDEMNIFY, HOLD HARMLESS South Bay Youth Basketball and its members, directors, board, spectators, participants, volunteers,
sponsors, agents, vendors, and the premises owners, operators, lessors and/or lessee including but not limited to their faculties, employees and
administrators (referred herein collectively for all purposes as “Releasees”) of and from any and all demands, actions, causes of action, claims of any
kind or nature whatsoever, known and unknown, anticipated or unanticipated, past or present, pertaining to the claim for injuries and damages which
is alleged to have been caused or alleged to be caused in whole or in part by the negligence or acts of the Releasees, the public, spectators,
referees, coaches, and/or player participants 1) which may exist arising out of or related to the participation in the basketball events and activities
sponsored by Releasees and/or 2) in connection with or relating to any property and/or equipment owned, maintained and/or operated by
Releasees.
I / we fully understand and acknowledge that: 1) there are risks and dangers associated with participation in the basketball events and activities
which could result in bodily injury, paralysis or death; 2) the social and economic losses and/or damages, which could result from these risks and
dangers described above, could be severe; 3) these risks and dangers may be caused by the action, inaction or negligence of the participant or the
action, inaction or negligence of others, including but not limited to the Releasees named herein; 4) there may be other risks not known to us or are
not reasonably foreseeable at this time.
I / we expressly accept and assume such risks and responsibility for the losses and/or damages following such injury, disability, paralysis or death
however caused and whether caused in whole or in part by the acts or failure to act and/or negligence of the Releasees named herein.
I / we hereby assign and grant to the South Bay Youth Basketball League the right and permission to use, publish and/or reproduce the
photographs/film/videotapes/electronic representations and/or sound recordings made of me by the South Bay Youth Basketball League. I hereby
release the South Bay Youth Basketball League from any and all liability from such use and publication and I specifically waive any right to any
compensation which I may have for any of the foregoing.
On behalf of the player and individually, the undersigned and/or legal guardian(s) for the minor participant executes this Agreement. If, despite this
release, the minor participant makes a claim against any of the Releasees, the parent(s) and/or legal guardian(s) will defend, indemnify and hold
harmless Releasees for any and all damages, attorneys fees, and costs incurred as a result of said claim.
I HAVE READ THIS WAIVER OF LIABILITY, RELEASE AND INDEMNITY AGREEMENT AND ASSUMPTION OF THE RISK AND FULLY
UNDERSTAND ITS TERMS. I EXPRESSLY ACKNOWLEDGE THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE
SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME. I INTEND MY
SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.

COACH'S CODE OF CONDUCT
SOUTH BAY YOUTH BASKETBALL
It is our intention to provide a league that promotes the utmost in fair play and good sportsmanship at
all times. We endeavor to provide a forum by which a child may learn valuable lessons in responsibility,
respect, fairness, team spirit, caring and good citizenship. All coaches must read and sign.
Coach’s Agreement
1. I will teach the players to be respectful of the rules of the game and to each other at all times. I will promote the
development of sound basketball fundamentals and the importance of team play.
2. I will foster a spirit of putting forth your best efforts rather then on just winning games.
3. I will demand that all players treat other players, coaches, officials with respect irregardless of race, creed, color,
sex or ability. I will observe all league rules and ensure fair playing time for all players.
4. The physical well-being of all players is paramount over winning games. I will review basic first aid principles and
promptly seek professional medical treatment of any injured player.
5. I will respect the judgment and integrity of all referees, scorekeepers, league officials and will not engage in any
abusive behavior or language at any time. I will encourage parents to stay in the stands during all games.
6. I will notify league officials promptly of any anticipated forfeitures of games due to lack of eligible players. I will
read and adhere to all the rules of the SBY Basketball league.
7. I understand any violations of this agreement either by a player, parents or me can result in penalties and
sanctions by league officials including, but not limited to fines, suspensions or the forfeiture of games & fees.
8. I understand that SBY Basketball adheres to the SAFEKIDS Program. I acknowledge that physical, mental, verbal
abuse or inappropriate language is forbidden at all times. I furthermore agree not to have any unsupervised,
social contact with my players and that any inappropriate touching is strictly forbidden.
