The following chart details the 18 states who currently receive funding from the CDC Disability and
Health (D&H) Branch and associated person(s) of contact, as well as sample local healthy communities
within these states and associated person(s) of contact who have been funded by the National
Association of Chronic Disease Directors (NACDD) or the National Center for Health, Physical Activity,
and Disability (NCHPAD).
The CDC D&H State Program Partners (e.g. the “state” applicants) are not required to partner with the
communities detailed in the NACDD or NCHPAD columns. NACDD offers this information to assist states
and communities with the collaboration process only, but does not require that state applicants use
these communities. States may collaborate with local communities not listed below.
