APPLICATION CHECKLIST
1.) APPLICATION FORM. Complete and sign the form.
2.) STATEMENT OF PURPOSE. On a separate sheet of paper, submit a brief, carefully worded
statement indicating your immediate and long-range goals, stating any areas of specific interest and
experience that may be relevant to your graduate program.
3.) LETTERS OF RECOMMENDATION. Obtain three recommendations from instructors
who have taught you, ideally, in the field of study for which you are applying. Letters may also be from
employers or supervisors who are in a position to compare your performance to that of your peers.
The letters may be mailed in a sealed envelope, with other application materials, or the recommenders
may send the letters directly to the Office of Graduate Admissions. Please note: Recommenders have
the option of completing the UMass Lowell recommendation form, or providing the letter on
professional letterhead.
4.) APPLICATION FEE. The application fee is $50.00. This fee is non-waivable and nonrefundable and must be received before your application can be processed. Apply online [credit

5.) OFFICIAL TRANSCRIPT. Provide an official transcript with appropriate bachelor degree
awarded. If applicable, please include any post-baccalaureate transcripts. Transcripts can be mailed
directly from your institution to the Office of Graduate Admissions. If you received your degree from
UMass Lowell, we will obtain your transcripts.
6.) OFFICIAL TEST SCORE REPORT(S). Request that the appropriate agency send an official test score
report for the GRE, GMAT or MAT (refer to individual department websites for program requirements).
Photocopies of your own personal score (examinees) report will not be accepted.
7.) INTERNATIONAL APPLICANTS. MUST ALSO PROVIDE OFFICIAL TEST SCORE REPORT OF
THE TEST of ENGLISH as a FOREIGN LANGUAGE (TOEFL). A waiver may be given to candidates
who have completed at least two semesters of full-time college/university work in the United States by
the date of submission of this application.
