HOW TO ENROLL INTO REEDLEY HIGH SCHOOL:
Attention NEW Students:
First you must pick up registration packet from either the Administration Office or the
Attendance Office. Make sure to complete the forms. When you bring the packet back to RHS,
you will need to bring with you the following Registration Requirements:
A. Verification of Birthday (Any 1 of the following, preferred in this order)

Current Passport
B. Proof of Residency (Any 2 of the following, preferred in this order)
1. Homeowners Mortgage Statement or Rental Agreement (Not rental receipt)
2. Current Utilities Bill (gas, electric, water, propane, trash (not telephone.) Note: PG&E will FAX
verification of new service if you have parent call them with a school site fax number.
3. Current Welfare Check Notice of Action
4. Affidavit of Residency notarized (Note-if this is used, 2 additional pieces of evidence named above still
need to be presented which can be either parties names (owner of live-in) listed on the affidavit).
5. Driver's License is NOT acceptable proof of residence.
6. Conditional Enrollment can be granted if only one proof of residency is available. Parent then has 30
calendar days to provide second proof. If none is given, student can be displaced by another student who
enters with all required registration requirements.

D. Immunization Record As Follows:
1. Polio (4 doses) - 3 doses meet requirement for ages 4-6 if at least one was given on or after the 4th
birthday.
2. Diphtheria, Pertussis, Tetanus - DPT (5 doses) -4 doses meet requirement for ages 4-6 if at least one
was on or after the 4th birthday.
3. Hepatitis B (3 doses)
4. Measles, Mumps, Rubella - MMR (2 doses)
5. PPD - TB skin test (within one year of entering school)
6. Varicella (1 dose) or health care provider - documented Varicella disease or immunity.
7. Physical exam for students entering 1st grade.
E. Guardianship
Students who are residing with a guardian must have evidence of that guardianship; or they must begin
proceedings with Fresno County. Information on acquiring guardianship is available at the District
Office, 675 W. Manning Avenue, Reedley, CA 93654.
