Welcome to the Florida Union Free School District community. In order to expedite the registration process
and facilitate a smooth transition for all new entrants, we have listed below all of the documents required to
complete the registration process. With the exception of Kindergarten registration, and/or in the instance of
an emancipated minor and/or unaccompanied youth the new entrant must accompany the parent or person in
lawful parental relation and/or legal guardian at the time of registration.

When a child’s parent(s), the person(s) in parental relation to the child or an emancipated or
unaccompanied youth requests enrollment in the school district, such child will be enrolled and shall
begin attendance on the next school day, or as soon as practicable, unless a determination of nonresidency is made on the date of such request for enrollment

Proof of parental relationship or proof that the child resides with the parent(s) or person(s) in parental
relation or is emancipated, should be provided at the time of registration. The District will accept
affidavits of the parents(s) and person(s) in parental relation or emancipated student indicating either:
(1) that they are the parent(s) with whom the child lawfully resides; or (2) that they are the person(s) in
parental relation to the child, over whom they have total and permanent custody and control, and
describing how they obtained total and permanent custody and control, whether through guardianship
or otherwise. The required affidavit forms are attached hereto and must be completed and brought
with the person(s) seeking a child’s enrollment in the District.
Proof of Residency and Parental Responsibility
 If you are a homeowner, you must provide a current tax bill or mortgage statement for your primary
residence (your name and address must appear on the bill)
 If you rent your home, you must supply a copy of a signed lease agreement with the term listed.
 If you rent without a lease, you must provide a letter from your landlord. (A landlord form will be
provided upon request). In addition, you should provide a current utility bill which lists your name and
the physical location of the student’s residence or the landlord’s statement must include an
explanation that you are not responsible for utilities. A post office box is not an acceptable
address.
 If you live with a district resident, and you do not pay rent, a signed statement should be provided by
the district resident and the parent or legal guardian of the registering student explaining the nature
and location of the living environment.

The District will consider other forms of documentation and/or information establishing physical
presence in the District and parental responsibility if submitted by the parent(s), persons in parental
relation and/or emancipated youth including, but not limited to the documents listed below:

Pay stub;

Income tax form;

Utility or other bills;

Membership documents (e.g., library cards) based upon residency;

Voter registration document(s);

Official driver’s license, learner’s permit or non-driver identification;

State or other government issued identification

Documents issued by federal, state or local agencies (e.g. local social service agency, federal Office
of Refugee Resettlement); or

Evidence of custody of the child in question, including but not limited to judicial custody or order of
guardianship documentation
Birth Certificate
 You must provide proof of your child’s age. A birth certificate should be provided if available.
 If you do not have a birth certificate, we will accept a baptismal certificate.
 If neither of the above documents are available, an United States or foreign passport are
acceptable.

Only in instances where none of the above documents are available to determine a student’s age, the
District will consider the following documents if the documents have been in existence for at least two
(2) years or more:
An official driver’s license
State or other government issued identification
School photo identification with date of birth
Consulate identification card
Hospital or health records
Military dependent identification card
Documents issued by federal, state, local agencies (e.g., local social service agency, federal
Office of Refugee Resettlement
(8) Court orders or other court-issued documents
(9) Native American tribal document or
(10)Records from non-profit international aid agencies and voluntary agencies

The District will NOT consider an Affidavit of Age.
Proof of immunization - (with office stamp and signature of health care provider)
A. 3 or more doses of diphtheria toxoid containing vaccine (DTP, DTaP).
B. 3 or more doses of oral poliovirus vaccine (OPV) or 4 or more doses inactivated polio vaccine
(IPV).
C. 1 dose of mumps vaccine administered on or after 12 months of age.

D. 1 dose of rubella vaccine administered on or after 12 months of age.
E. 2 doses of measles vaccine, first dose administered on or after 12 months of age.
(C, D, and E preferably as MMR)
F. 3 doses of hepatitis B vaccine.
G. 1 dose varicella (chicken pox) vaccine (students born on or after 1/1/98 or born or after 1/1/94 and
enrolling in the 6th grade) or physician signed document stating child has had chicken pox.
H. 1 dose of Tdap containing diphtheria, tetanus toxoids and acellular pertussis for students entering
6th grade or a comparable age level special education school or program and who are 11 years of
age or older. If your child received a Td, DT, or DTaP within the last two years, it is recommended
the Tdap be deferred until a period of 2 years has elapsed and you are advised by your family
health care provider.
Physical examination – New York State Law requires a physical examination for all new entrants to a district
as well as grades Kindergarten, 2, 4, 7 and 10.
Most Current Report Card
 (required for all middle and high school students, recommended for all other grade levels)
 Current Individualized Education Program (IEP)
 Any pertinent medical, social, psychological or academic records
Special Circumstances
 If you are not the natural parent, proof of legal guardianship is required. If you are the custodial
parent, a copy of the custody agreement must be presented at the time of registration.
Please read the section below that pertains to your situation to determine what documents
must be submitted:

IF YOU ARE A HOMEOWNER: You must provide either a copy of your most recent tax bill with the
section, block and lot number or a copy of a recent mortgage statement. The name and address on
these documents must match the name and address of the parent or legal guardian of the student
being registered. If you have just recently closed on a new home, please provide the Settlement
Statement from the closing or the Certificate of Occupancy or Deed, along with a homeowner’s
insurance policy.

IF YOU ARE A RENTER, you must supply a copy of your signed leased agreement with the term
listed.

IF YOU RENT WITHOUT A LEASE, (on a month to month basis) the following must be provided:
o A letter from the landlord (a blank form will be provided) and
o A Cable, Electric, or Fuel/Oil Bill, clearly stating the name and physical address of the
registering family.

IF YOU LIVE WITH A DISTRICT RESIDENT: and do not pay rent, a signed statement, which will
be provided, must be completed as described below:
o It must be first signed by the district resident who must also provide proof of residence as
stated above.
o It must also be signed by the parent or legal guardian of the registering student.
The District will also consider the following evidence of residency if submitted by the parent(s) or persons
seeking enrollment:

papers.
As soon as practicable and not later than three business days after a child’s initial enrollment, the parent(s),
the person(s) in parental relation to the child or the child, as appropriate, must submit any and all required
and appropriate documentation and/or information in support of the child’s residency in the district. The Board
of Education or its designee shall review all such documentation and/or information and make a residency
determination by not later than the third business day following a student’s enrollment; provided that if such
documentation and/or information is submitted on the third business day after initial enrollment, the Board of
Education or its designee in its discretion may make the residency determination no later than the fourth
business day after initial enrollment.
Please note: when a currently enrolled student has a change of address, or transfers within the
school district, residency MUST be verified again. There will be NO exceptions. All mail will be sent
to the previous address, and transportation will not be arranged until proper proof of residency is
obtained.
