Job Purpose
To provide short term support to service users who are experiencing a mental health crisis
and find themselves in need of short term 24 hour staffed supported accommodation

Key Duties












To act as a key worker for a number of residents within the project, carrying out
needs assessments and formulating support plans, risk management plans
To provide appropriate short term recovery based support.
To undertake therapeutic interventions aimed at resolving the current crisis and
communicate hope.
To assess the needs and risks as relevant to each Service User and to support them
to set and achieve their identified outcomes.
To monitor and record the Service User’s progress by a process of regular review.
To signpost people to a range of services and organisations, facilitating liaison
between service users and other agencies.
Accurately maintain all service user recording systems paper based and
computerised to enable monitoring & reviewing
To Participate in service user reviews and provide monitoring information as
required.
To promote collective responsibility and active participation in the Project milieu
To facilitate resident meetings, therapeutic groups/activities.
To Carry out daily household chores
To work within policies and procedures; in particular Health and Safety, Lone
Working, Confidentiality, Professional Boundaries, Disciplinary Rules and Health and
Care Professional Council ‘Standards of conduct, performance and ethics.

To carry out, record and review assessments and risk management strategies for
each service user.
To monitor work areas to ensure safety of self and others.

themselves at risk.
To record and report all accidents and significant incidents.
To evacuate buildings in an emergency.
To ensure service users are encouraged to report any risks or concerns of risk.
