Due to a variety of district regulations and policies, we ask for your cooperation in completing this ‘onestop’ permission slip for tennis. Please review each category separately. Mark those permissions that you
authorize and sign at the bottom. If you have any questions or concerns, feel free to contact us at the

2. Has my permission to drive to the following locations to participate in team practices or
matches: Fort Vancouver HS, Hudson’s Bay HS, Columbia River HS, Vancouver Tennis and
Racquetball Center, and Green Meadows Athletic Club. I understand that the above-named
student will not transport other player unless legally entitled to do so and that the above-named
student assumes all liability and responsibility for such travel. Students are not allowed to use
personal vehicles to travel to or transport fellow players from matches outside the Vancouver
School District.
3. My child has permission to be transported home with the following parents. Even if
transporting your own child, all players leaving an away match must be signed out with the coach.
If the name is not specified below, players will not be allowed to leave except with the team bus.
