Have your say on Primary Care in Buckinghamshire Bringing clinical leadership to local health needs www.aylesburyvaleccg.nhs.uk Chief Officer: Louise Patten Clinical Chair: Dr Graham Jackson Primary Care What does it mean to you? • Aylesbury Vale & Chiltern Clinical Commissioning Groups are to produce a Buckinghamshire wide primary care strategy focused on the needs of our locality populations. • For the purposes of the strategy ‘Primary Care’ does not just mean General Practice, but all services accessed outside of hospital including pharmacy and adult community health teams regardless of the time accessed day, night or at weekends. • We ask that patients and the public register on ‘www/letstalkhealthbucks.nhs.uk’ to respond to our simple questions. • You will have this opportunity to share your views until 28th November 2014 but in January you will have another opportunity to tell us what you think when we move to a period of formal consultation on the drafted strategy document. Why do we need to change / build a strategy for primary care nationally? • Service delivery and economic pressures on practices including changes to their contracts • Shortfall and pressures on the GP workforce • Need to integrate with social care to manage an ageing population • Refocus on more prevention and proactive case management • Availability of 7 day services • Reducing variable quality, better diagnosis and treatment • Inadequate premises • Requirement for Care Quality Commission (CQC) inspection and registration Older People over 75yrs Increasing emergency hospital admissions Growth in chronic disease and long term conditions Why do we need to change / build a strategy for primary care locally?
