Permanent senior level position at FLSmidth Minerals in the Engineering sector

Duties, Responsibilities & Achievements
I am primarily responsible for conducting regular SHE audits and inspections at
the construction sites where our company has contractors doing construction
and installation work for us. I assist the Project Managers with compiling SHE
files for the upcoming jobs we tender for and assist them also with the planning
and execution of the SHE requirements and specifications on the various sites. I
also assist the Safety Manager with the record keeping of the SHE statistics of
the various sites.
From 2 May 2007 to 31 December 2012 I worked as the SHE Officer for
FLSmidth on the then Potgietersrust Platinums 600ktpm Concentrator Project,
35km Northwest of Mokopane, in Limpopo, South Africa. We had 2
subcontractors working for us on site, 1 on the mill installation and another on
the primary crusher installation. I was responsible for ensuring that these
subcontractors worked according to the rules of the client and the acts and
regulations of the country.
During this period, we started on the PPC Dwaalboom 3300tpd Expansion
Project where we were responsible for the erection of the major portions of a
new cement plant. We erected and equipped the Raw Mill, Pre-heater, Kiln,
Cooler and Coal Mill. Again, here we utilised the skills of several subcontractors
during the project. I visited the site once every 3 weeks to do an internal SHE
audit on the contractors and even spent 3 months on site during February to
April 2008, when PPC required the full-time presence of an FLSmidth Safety
Officer on site, although each of our subcontractors had their own Safety Officer
on site.
During 2008 we started with the Anglo Coal: Mafube expansion project where
we were responsible for the installation of a Stacker / Reclaimer. Here we also
utilised the services of subcontractors with their own Safety Officers and I had
to visit the site once every 6 weeks to do a SHE audit on them.
In 2010 we started with a large contract on Anglo Coal: Phola Coal Processing
Plant Project. Here we were responsible for supplying and installing 4 Stackers
and 3 Reclaimers. Again we used subcontractors to do the work and there was
a permanent Safety Manager on site, overseeing the work of 4 Safety Officers,
working at the various locations on site. I was again just responsible for visiting
this site once in 3 weeks to do an internal SHE audit and also to liaise with the
client on matters of concern and when there were any complaints about our
subcontractors.
While we were still busy on Phola, another project started at Balfour, Gauteng,
where we were responsible for supplying and installing 2 mills for the Great
Basin Gold Limited – Burnstone Metallurgical Plant. As usual, we utilised
subcontractors to do the work and apart from the compiling of the SHE files for
this project, I again only had to visit the site once in 3 weeks to do an
inspection and /or SHE audit.
During 2010, the planning phase for a large project at Exxaro’s Grootegeluk
Mine Beneficiation Plant, kicked off. I was involved with this project from the
beginning and attended all the planning meetings with the client. I was
responsible for compiling the SHE files for this project and to make sure all the
arrangements regarding Safety and Health was made for the site
establishment. We appointed a contractor as our Safety Manager on site and he
was responsible for managing the Safety Officers of the 4 subcontractors we
utilised. Since the site establishment in March 2011, I was responsible for
visiting this site once every month to do an internal SHE audit and inspection
on our system and to make sure our Safety Manager did his job. FLSmidth has
won the Safety Trophy on this site every month so far.
From June 2010, I was responsible for the Safety of the Mobile Crusher Plant
erection at Rustenburg Platinum: Mogalakwena South Concentrator. We were
responsible for supplying and installing a mobile crusher plant that consisted of
a loading box with a vibrator feeder, feeding a conveyor belt that travels past
underneath an electromagnet, then tips into another loading box with 2 vibrator
feeders, each feeding its own conveyor belt that supplies broken rock to a cone
crusher unit. The underflow of these cone crushers were taken by conveyor belt
to the main plant and the overflow was taken back to the loading box where it
came from. I was permanently on site at least 3 days per week and had to
make sure the subcontractors that did the work for us, did it according to the
client’s specifications and the legal requirements for Safety and Health. When
the installation and erection of the plant was completed and the plant
commissioned in February 2011, I had to make sure the FLSmidth employees,
who were responsible for the operation and maintenance of the plant, knew the
SHE requirements of the client and their Safety Officer can take over from me,
knowing without a doubt that he can do his job correctly.
From the beginning of 2012 I have been extensively busy with compiling SHE
files for various projects and jobs currently in progress, or being planned for.
Presently we have a labour force of just over 500 permanent employees and
our average 12 month rolling Lost-time Injury Frequency Rate (LTIFR) is 0 per
200 000 man hours, Reportable Injury Frequency Rate (RIFR) is less than 0 per
200 000 man hours and our Fatal Injury Frequency Rate (FIFR) is 0 per 200
000 man hours.

I started here as a Shiftboss, responsible for the daily production of broken ore,
in a safe, healthy and cost-effective manner. I was in charge of 4 development
cleaning crews of 6 men each and a tramming crew of 6 men. I was later
transferred to the position of Risk Assessment / Safety Officer due to the
growing needs of the mine. My duties included: Safety Inspections in all the
surface operations of the mine and the underground Mechanised Section;
facilitating the necessary risk assessments when the need arose; keeping
records of all the accidents that happened on the mine, typing all the required
data into the computer on a daily basis; sent the required data on the mine
accidents to the DME monthly; kept the filing system of all the fatal accidents
up to date and assisted with compiling the fatal files for the DME enquiries;
purchased all the stationery required by the Safety Department; delivered
safety training to all the ex-leave and new employees and contractors on a
weekly basis; assisted the Safety Manager with compiling the month-end,
quarterly and year-end reports, using the stats to create the necessary graphs;
developed monthly safety talk topics and posters, based on the safety statistics
of the previous month; assisted with all incident and accident investigations in
my area of responsibility, using the RCAT principle and assisted with the
development of Safe Work Procedures (Mine Standards).
We utilised a labour force of 2500 per month on average and our average 12
month rolling Lost-time Injury Frequency Rate (LTIFR) was 8.33 per million
man hours, Reportable Injury Frequency Rate (RIFR) was 5.21 per million man
hours and our Fatal Injury Frequency Rate (FIFR) was 0.02 per million man
hours.
