What is Florida SHOTS?
• Florida SHOTS is a confidential, computerized information
system that stores vaccination histories.
• The system helps enrolled healthcare providers, schools,
and daycares keep track of immunization records.
• It also generates the Form 680, a certified electronic
version of Form 680, and reminder/recalls.
Who is in Florida SHOTS?
Florida SHOTS includes:
• All children born in Florida since January 2003
• Patients receiving services from a CHD in the state
• Children and adults who receive immunizations from any of
nearly 3,000 private physicians currently participating in
Florida SHOTS

How can Florida SHOTS help you?
• Florida SHOTS produces the legal immunization record
(Form 680) required for school attendance at the click of a
button. (Printed and electronically certified versions are
available.)
• Immunization information is available 24 hours a day, 7
days a week.
• The registry also produces reminder/recall messages and
assessment levels, which help improve immunization
coverage rates.
• Registries can help kids and doctors in case of natural
disasters.
How can Florida SHOTS help you?
Electronically Certified Form 680s
• Can be accepted as proof of immunization by enrolled
schools and licensed childcare providers
• Can be accepted on white paper
• Schools and daycares can access electronically certified
copies from Florida SHOTS
• Parents can access valid copies by using a Parent PIN
number
Setting Preferences to Produce Certified 680s
• Local Organization Administrator must select “Personnel
List” under “Administration” in the left menu. On the
“Personnel Maintenance” screen:
o Set up permissions for authorized individuals whose names will be
used to electronically sign the new 680 Forms (“Certify Form
680”).
o Set up permissions for users who will need access to issue the
certified forms (“Create Certified Form 680”).

Print,” “Certify Only”
• For certify options, you must select the “Physician or
Authorized Signature” to appear on the 680 from the
provided drop-down list (names to whom your Local
Organization Administrator has given permission to
certify a Form 680).

• Data upload is a process that exports immunization
information from current electronic files and transfers it into
the Florida SHOTS registry.
• As of April 2013, we’ve uploaded more than 145 million
shots into the registry!
