LIABILITY RELEASE FORM
All persons touring the USD facility must complete a Liability Release as a condition of
attending the tour.
All attenders MUST DownLoad the form from the course WebPage, Fill in the Blanks, and
SIGN the form. The attenders MUST BRING the completed form to the USD facility and give
the form to the USD personnel prior to formal admission to the USD Operation.

Travel Arrangements
Students are responsible for making their OWN travel arrangements. At the conclusion of the
tour, any student is free to go his or her own way. Students do NOT need to return to campus.
Field Trip Grading



5 points for attendance. Completion of the trip report will be considered evidence of
attendance
0-5 Points for sending to the Main Host a THANK YOU eMail within one week of the
tour
o Thank-you-note Grading based on including a THOUGHTFUL comment on the
tour and how much you learned.
 Examples of grading schema
 A “thanks for the Tour” eMail gets about 1-point
 A PARAGRAPH that lists specific activities, explanations, etc.,
demonstrations that were of interest to you gets 5-points
 Attach a copy of your eMail Thank-You as an APPENDIX to your trip
report
