Page | 2 ABOUT US Founded in 2007 by six health centers and the Community Health Care Association of New York State (CHCANYS), HCNNY today has grown to serve 18 health centers across the eastern U.S. Our data reporting and benchmarking solutions contain demographic and clinical information on nearly Page | 3 260,000 patients served network-wide. Our mission is to help members leverage eClinicalWorks (eCW) so they can provide quality, lowcost healthcare to the communities they serve. We offer support, training, reporting services, and ongoing collaboration opportunities focused on maximizing the use of health centers’ eCW systems. A board of directors made up of executives from member centers provides us with oversight and accountability, while our operations are led by clinical, finance, and IT committees that meet regularly to prioritize and share best practices. Meet our staff Sandy Cafarchio, Executive Director Sandy leads the effort to help HCNNY members and customer health centers leverage their investments in health information technology (HIT). She has a passion for the mission of health centers, because of the patients they serve. Sandy joined the organization in 2008 as Project Director to manage the newly formed network at the Community Health Care Association of New York State (CHCANYS). When HCNNY was incorporated in 2010, she was promoted to the position of Executive Director. Sandy oversees all HCNNY activities, recruits and develops staff, and supports the Board of Directors in its plans. She consistently fosters member trust and encourages an active and participatory Board to maintain compliance and fiscal integrity. She previously served as the Chief Information Officer and Finance Director for an FQHC, where she successfully implemented and managed early technology solutions for practice management and communication systems, and oversaw finances and grants. Outside the office, Sandy spends her time with her children and grandchildren, and enjoys international travel. Since joining HCNNY, she has successfully designed, implemented, and managed HIT infrastructure and support solutions in alignment with HCNNY’s strategic plan. Her duties include project management, Meaningful Use support, Patient Centered Medical Homes (PCMH) coaching, on-site eCW training, and data center/disaster recovery support for our members and external clients. Under her direction, HCNNY has evolved into a provider of a highly available EHR and reporting hosting solutions and has expanded training and technical assistance programs to meet the evolving demands of our members and fee-for-service customers. She also supports HCNNY’s member committees and helps those committees devise and execute their respective plans, as well as identify best practices to share with each other. Page | 4 Before joining the HCNNY, Stephanie was Senior Director of Information Systems and Billing for the Washington University Academic Medical Practice in St. Louis, where she supported more than 1,100 physicians with their practice management and EHR systems. In her spare time, Stephanie enjoys hiking, biking, snowboarding, traveling, and spending time with Page | 5 family and friends. (QUOTE) “I enjoy working with people to help them simplify processes and achieve their goals.” Desiree Railine, HIT Implementation Specialist/Trainer Desiree joined HCNNY in 2010. In her position, she works with health centers by performing business and clinical workflow analyses. She makes recommendations that can help those health centers implement the most effective product features within their eCW EHR. Desiree also develops, coordinates, and leads the effort in member training, from new modules and upgrades to new implementations. She also engages with health center staff to help them meet requirements for programs such as Meaningful Use and PCMH. Desiree is proud that her work makes a difference in the lives of others. She grew up around relatives in healthcare—from nurses to midwives to administrators. So, healthcare was a natural career choice for her. When she’s not working at HCNNY, Desiree enjoys reading, painting, and spending time with her granddaughter. I have been very blessed in my lifetime; and I want to be able to be a blessing to others.” Lisa Szymaszek, Administrative Assistant Lisa joined HCNNY in 2010. She keeps the office humming, from scheduling meetings to taking minutes and planning events. She also manages HCNNY’s payroll. Lisa previously worked for a nonprofit in Delmar that advocated for people with developmental disabilities. She enjoys contributing to improving the health of the underserved. When she’s not working at HCNNY, Lisa enjoys photography and travel, and is active with veterans as an auxiliary member of both the VFW and American Legion. Michelle Tropper, Clinical Quality Improvement Coordinator Michelle joined HCNNY in 2015. She leads the organization’s efforts in key quality improvement (QI) activities and coordinates projects with health centers and other partners. Before she joined HCNNY, Michelle worked at Perlmutter Cancer Center, part of NYU Langone Medical Center, where she led the expansion of community-based outreach, research, and educational initiatives for patients and their families. Previously, she worked at the American Cancer Society, Eastern Division. Among her accomplishments, Michelle created continuing medical education initiatives to increase cancer screenings in underserved populations for both the New Jersey Academy of Family Physicians and the New York State Academy of Family Physicians. Outside the office, Michelle enjoys nurturing the musical pursuits of her two daughters. She’s also an avid public speaker and a volunteer for Girl Scouts and the American Cancer Society. She holds a Master’s degree in Public Health from Rutgers School of Public Health. If you’re new to eCW, we can help you train your staff on the system right from implementation. And if you’ve been using eCW for a while, we can work with you on a one-time or an ongoing basis. We design customized training services to meet the needs of our customers and members as they arise, and deliver standardized training on an ongoing basis to members as new hires and new upgrades require fresh training. (HEADER) Patient Centered Medical Home (PCMH) (LEAD DESCRIPTION) The NCQA requirements for PCMH can be complex and confusing. HCNNY understands how important accreditation is to your health center. Our coaching service can bring clarity and understanding to the process of meeting the PCMH requirements. And help you with documentation requirements prior to submission. Our educational support helps you understand NCQA requirements to achieve PCMH recognition. Here’s how the process works. 1) First, we’ll perform a gap assessment that takes you through each requirement, identifies what your current process is today, and what processes you would need to modify. 2) Next, we’ll use the gap assessment to discover if you have any existing reports that you could use to track the requirements—and how you could create reports to monitor the various requirements. 3) We’ll coach you on screen shots and other supporting documentation required. 4) Once you have all of your necessary documentation, we’ll advise you on how to upload the material to the NCQA website and provide the additional explanations. 5) Finally, we’ll review the submission package with you before you submit it.
