CONTRACT REAPPOINTMENT EVALUATION DOSSIERS REQUIRED MATERIALS, FORMAT & SUBMISSION INSTRUCTIONS These guidelines have been adopted to prescribe the content and format of Contract Reappointment Evaluation Dossiers. These guidelines are intended to ensure that Contract Reappointment Dossier materials are transmitted in a consistent format to aid in efficient review and decision making. Please ensure that all requirements are met before submitting the dossier. Dossiers that do not meet these requirements will be returned. There are three sections to this document: 1. Description of Dossier Materials 2. Organizing, Formatting & Naming Dossier Materials 3. Submitting the Dossier Questions regarding these guidelines should be addressed to the Office of Faculty Affairs at 770.3247 or 770.3261. DESCRIPTION OF DOSSIER MATERIALS 1. Self-Evaluation Candidate submits to Faculty Affairs as part of Rank Promotion Dossier by October 1. The self-evaluation serves as the foundation of the dossier and contract reappointment evaluation. Using the Self-Evaluation for Contract Reappointment Evaluation template, the faculty member will first provide his/her Philosophy of Teaching Statement. He/she will next provide a self-assessment of his/her activities and performance for the contract period under review. This self-assessment should include an overview of teaching, creative activity/research, and service. 2. Curriculum Vitae (CV) Candidate submits to Faculty Affairs as part of Rank Promotion Dossier by October 1. The most recent version of the faculty member’s CV. It must be dated and paginated. It should include a list of courses taught, a list of administrative assignments and professional creative activity/research. 3. Student Course Evaluations Candidate submits to Faculty Affairs as part of Rank Promotion Dossier by October 1. The faculty member’s Student Course Evaluations for the contract period under review. The faculty member should check with the Dean’s Office if he/she does not have copies of his/her student course evaluations for the contract period under review. 4. Syllabi Candidate submits to Faculty Affairs as part of Rank Promotion Dossier by October 1. The faculty member’s syllabi for courses taught during the contract period under review. 5. Annual Evaluations Candidate submits to Faculty Affairs as part of Rank Promotion Dossier by October 1. Contract Reappointment Evaluation Dossier Content & Format Page 1 of 5 Revised 9.29.15 The faculty member’s Annual Evaluation/Update of Professional Activities Form, Parts I & II for each year of the contract period under review. The faculty member should check with the Dean’s Office if he/she is missing either Part I or Part II of the Annual Evaluation. 6. Supplemental Support Candidate may submit to Faculty Affairs to be included in Rank Promotion Dossier at any time. This includes any additional information in support of the review. This item can be updated at any time. However, materials submitted after a stated dossier submission deadline does not affect subsequent deadlines and does not cause the Peer Committee, the Dean or the Provost to be obligated to re-evaluate a faculty member. 7. External Reviewer Nomination Form* Required for 5-year and 10-year contracts. Candidate submits to Dean by September 15. Office of the Dean submits to Faculty Affairs via facultyaffairsforms@uncsa.edu by February 8. The External Reviewer Nomination Form is completed by the Candidate and submitted to the Dean by September 15. Candidates should recommend at least five reviewers to their dean. See Guidelines for External Reviews for Contract Reappointment Evaluation and External Reviews FAQ for more information about selecting external reviewers and the external review process. The Dean submits the External Reviewer Nomination Form to Faculty Affairs by February 8 for inclusion in the Rank Promotion Dossier. 8. Copies of all Request for External Review Letters* Required for 5-year and 10-year contracts. Office of the Dean submits cc to Faculty Affairs via facultyaffairsforms@uncsa.edu as letters are sent to potential external reviewers. The Dean submits copies of all letters sent to potential external reviewers requesting external review regardless of whether external reviewer accepted or not. The Request for External Review for Contract Reappointment Evaluation Letter Template must be used for all letters of request sent to potential external reviewers. 9. Final List of External Reviewers* Required for 5-year and 10-year contracts. Office of the Dean submits to Faculty Affairs via facultyaffairsforms@uncsa.edu by February 8. The Final List of External Reviewers is completed by the Dean and sent to Faculty Affairs for inclusion in the Contract Reappointment Evaluation Dossier. 10. External Review Letters* Required for 5-year and 10-year contracts. Faculty Affairs sends to Deans and uploads to Contract Reappointment Evaluation Dossier as received. External Reviewers submit the External Review for Contract Reappointment Evaluation and narrative evaluation to Faculty Affairs via facultyaffairsforms@uncsa.edu. Faculty Affairs forwards these documents to the Candidate’s dean and uploads them to the Candidate’s Rank Promotion Dossier as they are received. Peer Review Committee Evaluation & Recommendation The Peer Review Committee Chair submits to Faculty Affairs via facultyaffairsforms@uncsa.edu by December 10. The Peer Review Committee submits the Peer Review Committee Evaluation & Recommendation for Contract Reappointment Evaluation to the Office of Faculty Affairs. This document addresses: the three areas of evaluative criteria: creative activity/research, service, and teaching. The Peer Committee should indicate their assessment of the faculty member in each area according to the guidelines outlined in the UNCSA faculty manual Peer Review Committee members’ classroom observations (e.g. classroom, studio, lab) student evaluations for each year under review More information regarding the Peer Review Committee evaluation can be found in Section 8.0, Employment Policies & Procedures for Faculty of the UNCSA Faculty Manual. 12. Dean’s Evaluation & Recommendation The Office of the Dean submits to Faculty Affairs via facultyaffairsforms@uncsa.edu by February 8. The Dean submits the Dean’s Evaluation & Recommendation for Contract Reappointment Evaluation to the Provost. This document addresses: the three areas of evaluative criteria: creative activity/research, service, and teaching. The dean should indicate his/her assessment of the faculty member in each area according to the guidelines outlined in the UNCSA faculty manual the dean’s instructional observations (e.g. classroom, studio, lab) student evaluations for each year under review recommendations and expectations for future growth and development More information regarding the dean’s evaluation can be found in Section 8.0, Employment Policies & Procedures for Faculty of the UNCSA Faculty Manual. 13. Acknowledgment Statement (Upload by February 8) The Office of the Dean submits to Faculty Affairs via facultyaffairsforms@uncsa.edu by February 8. The Acknowledgement Statement is the Faculty Member’s acknowledgement that he/she has reviewed the completed Contract Reappointment Dossier and discussed the evaluation with the Dean. The Office of the Dean must complete the Acknowledgement Statement for Contract Reappointment Evaluation and have it available for the faculty member’s signature when the Dean meets with the faculty member to discuss the evaluation. ORGANIZING, FORMATTING & NAMING DOSSIER MATERIALS CANDIDATES: What follows is the complete order of the Contract Reappointment Evaluation Dossier. Candidates need only be concerned with those documents followed by (C). Please adhere to the following standards: Dossier materials must be saved to the Contract Reappointment Evaluation Dossier flash drive in the order shown below. The flash drive is pre-formatted with the appropriate folders. Documents must be saved in pdf format. Order of Contents Self-Evaluation (C) Self-Evaluation Curriculum Vitae (C) CV Student Course Evaluations (C) Course Evals, Course #, Term (e.g., Course Evals MST1100 S15) Each set of course evaluations for a course should be saved as a separate document. Syllabi (C) Syllabus, Course #, Term (e.g., Syllabus HUM2101 F15) Each syllabus should be saved as a separate document. Annual Evaluations (C) Annual Evaluation, Year (e.g., Annual Evaluation 2014-2015) Part I and Part II for each academic year should be saved as one document. Each academic year should be saved as a separate document. Supplemental Support (C) Each document should be saved separately. There are no title requirements but titles should be succinct and clear with the reviewer’s eye in mind. Document titles that exceed our space requirements will be returned to the faculty member for editing. External Review Documents External Reviewer Nomination Form (C), (D) External Reviewer Nomination Form o This document is submitted by the Candidate to the Dean. The Dean is responsible for sending the document to Faculty Affairs for inclusion in the dossier. Requests for External Review (D) Request Letter, Last Name of Reviewer (e.g., Request Letter Jackson) Each request letter should be saved as a separate document. Peer Review Committee Narrative Evaluation Letter (P) Peer Review Committee Narrative Evaluation Letter Dean’s Narrative Evaluation Letter (D) Dean’s Narrative Evaluation Letter SUBMITTING THE DOSSIER Candidate Materials: Faculty Affairs will provide a Contract Reappointment Evaluation Dossier flash drive to faculty undergoing contract reappointment evaluation. The flash drive will be pre-formatted with the required dossier folders. Candidate materials should be saved to the appropriate folder in the Contract Reappointment Evaluation Dossier as described earlier in this document under Description of Dossier Materials and Organizing, Formatting & Naming Dossier Materials above. Contract Reappointment Evaluation Dossiers must be submitted to the Office of Faculty Affairs by October 1. The one exception is the External Reviewer Nomination Form. It should be submitted to the Candidate’s dean by September 15. The dean will submit the form to Faculty Affairs by February 8. Dean & Peer Committee Materials: Deans and Peer Committee Chairs should submit dossier materials to Faculty Affairs via facultyaffairsforms@uncsa.edu.
